摘　　要：目的:初步探讨女性神经性厌食(anorexia nervosa,AN)患者不同亚型,即限制型(anorexia nervosa restricting type,ANR)和暴食/清除型(anorexia nervosa binge-purging type,ANBP)临床及心理特征的差异。方法:采用进食障碍问卷(eating disorder questionnaire,EDQ)及进食障碍调查量表-1(eating disorders inventory-1,EDI-1),对269例ANR和151例ANBP女性AN患者进行横断面调查。对其人口学资料、体质量指数(body mass index,BMI)、EDI-1量表因子分进行比较;采用logistics回归分析,探讨可能影响AN分型的因素。结果:ANBP组当前BMI高于ANR组,但其期望的BMI却低于ANR组。ANBP组EDI-1总分及各因子分均高于ANR组。Logistics回归显示暴食、无效感分值越高,越倾向于ANBP亚型;成熟恐惧分值越高,越倾向于ANR亚型。结论:暴食、无效感、成熟恐惧对女性AN的诊断分型具有预测作用;ANBP与ANR亚型可能具有不同的心理特征。Objective:To explore clinical and psychological characteristics of anorexia nervosa restricting type(ANR) and anorexia nervosa binge-purging type(ANBP) subtypes of female anorexia nervosa(AN).Methods:A total of 269 ANR and 151 ANBP female patients were recruited and instructed to complete Eating Disorder Questionnaire(EDQ) and Eating Disorder Inventory-1(EDI-1).The demography traits,BMI and EDI-1 factors scores were respectively compared between ANR and ANBP patients.Furthermore,the influencing factors to sub-typing of anorexia were analyzed with logistics regression.Results:The statistical difference in demography traits was not found between two subtypes of AN after age-matched.The ANBP group had higher BMI but lower expected BMI than the ANR group.The total score and all sub-scales scores of EDI-1 were significantly higher in ANBP group than in the ANR group.Logistics regression results showed that bulimia,ineffectiveness and maturity fears were the significant predictors of subtypes of female anorexia,namely,higher scores of bulimia and ineffectiveness were associated with ANBP subtype,but higher scores of maturity fears was associated with ANR subtype.Conclusions:It suggests that bulimia,ineffectiveness and maturity fears may be powerful predictive factors to sub-typing of female anorexia nervosa,and the psychological characteristics associated with eating disorders are different between ANBP and ANR patients.
