Er:YAG激光在中重度慢性牙周炎辅助治疗中的临床评价

摘　　要：目的评价Er:YAG激光在中重度慢性牙周炎辅助治疗中的临床效果,为Er:YAG激光在牙周炎中的辅助治疗提供理论依据。方法20例中重度慢性牙周炎患者,将患者的左右半口按随机数字表法分为实验组和对照组。实验组行超声龈下刮治器械治疗,后行Er:YAG激光辅助治疗,对照组行超声龈下刮治器械治疗。比较两组治疗后6、12周牙周袋探诊深度(PD)和临床附着丧失水平(CAL)、牙龈探诊出血(BOP)的变化。结果符合统计的位点为823个,其中实验组418个位点,对照组405个位点。治疗前,治疗后6、12周,实验组PD分别为(6.85±1.62)、(3.92±0.74)、(3.47±0.71)mm, CAL分别为(6.71±1.50)、(5.02±0.69)、(4.53±0.56)mm, BOP阳性率分别为79.9%、46.7%、23.9%;对照组PD分别为(6.78±1.54)、(4.05±1.12)、(3.70±0.57)mm, CAL分别为(6.62±1.40)、(4.89±1.18)、(4.84±1.09)mm,BOP阳性率分别为82.7%、44.9%、37.0%。治疗后6、12周,两组PD、CAL和BOP阳性率与治疗前相比均有明显下降,差异具有统计学意义(P<0.01)。治疗后6周,两组PD、CAL和BOP阳性率比较差异无统计学意义(P>0.05);治疗后12周,两组PD、CAL和BOP阳性率比较差异均有统计学意义(P<0.01)。结论在合适的功率及规范操作下, Er:YAG激光对中重度慢性牙周炎患者进行辅助治疗是安全有效的。Objective To evaluate the clinical effect of Er:YAG laser in the adjuvant treatment of moderate and severe chronic periodontitis, so as to provide theoretical basis for the adjuvant treatment of periodontitis with Er:YAG laser. Methods There were 20 patients with moderate and severe chronic periodontitis,and their left and right half were divided into experimental group and control group according to random number table method. The experimental group was treated with ultrasound subgingival curettage instruments, followed by Er:YAG laser-assisted therapy, and the control group was treated with ultrasound subgingival curettage instruments. Comparison were made on changes of probing depth(PD), bleeding on probing(BOP), clinical attachment level(CAL) after 6 and 12 weeks of treatment in two groups. Results There were 823 loci accorded with the statistics, with 418 loci in the experimental group and 405 loci in the control group. Before treatment,after 6 and 12 weeks of treatment, the experimental group had PD respectively as(6.85±1.62),(3.92±0.74) and(3.47±0.71) mm, CAL respectively as(6.71±1.50),(5.02±0.69) and(4.53±0.56) mm, and BOP positive rate respectively as 79.9%, 46.7% and 23.9%. The control group had PD respectively as(6.78±1.54),(4.05±1.12)and(3.70±0.57) mm, CAL respectively as(6.62±1.40),(4.89±1.18) and(4.84±1.09) mm, and, BOP positive respectively as 82.7%, 44.9% and 37.0%. After 6 and 12 weeks of treatment, both groups had lower PD, CAL and BOP positive rate than those before treatment, and their difference was statistically significant(P<0.01). After6 weeks of treatment, both groups had no statistically significant difference in PD, CAL and BOP positive rate(P>0.05). After 12 weeks of treatment, both groups had no statistically significant difference in PD, CAL and BOP positive rate(P<0.01). Conclusion Under appropriate power and standard operation, Er: YAG laser is safe and effective in the adjuvant treatment of moderate and severe chronic periodontitis.
