摘 要:目的:分析MSCT对胃肠道和胃肠道外间质瘤的影像学特点及其临床诊断价值。方法:搜集2009年--2018年20例经手术病理证实的腹部间质瘤CT资料,对各部位间质瘤的大小、形态、密度、生长方式、是否坏死囊变、钙化、强化方式与程度、是否转移以及肿瘤恶性危险度分级进行分析,并与手术与病理对照。结果:20例间质瘤发生于胃8例,发生于小肠9例,发生于胃肠道外3例;肿瘤最大径1.5-20.5㎝;肿瘤呈类圆形11例,不规则形9例,病灶与周边组织边界不清11例,边界清晰9例;肿瘤密度均匀5例,不均匀15例;液化坏死16例,伴出血8例;增强扫描均匀强化3例,不均匀强化17例;轻度强化8例,中度强化5例,明显强化7例。高危险度7例,中危险度6例,低危险度7例。结论:通过分析,CT能有效诊断腹部胃肠道来源和胃肠道外来源的间质瘤,并能为肿瘤的病理分级、危险程度的区分提供依据,是诊断间质瘤有价值的检查方法。Objective:to analyze the imaging features and clinical diagnostic value of MSCT in gastrointestinal and gastrointestinal external stromal tumors.Methods:the CT data of 20 cases of abdominal stromal tumors confirmed by surgery and pathology from 2009 to 2018 were collected. The size, shape, density, growth pattern, necrotic cystic change, calcification, enhancement mode and degree of stromal tumors were analyzed. Metastasis and malignancy grade were analyzed and compared with operation and pathology.Results:20 cases of stromal tumors occurred in the stomach in 8 cases, in the small intestine in 9 cases and in the outside of the gastrointestinal tract in 3 cases. The tumor was round in 11 cases, irregular in 9 cases, unclear in 11 cases and clear in 9 cases, homogeneous in density in 5 cases, uneven in 15 cases, liquefaction necrosis in 16 cases and hemorrhage in 8 cases. Homogeneous enhancement in 3 cases, uneven enhancement in 17 cases, mild enhancement in 8 cases, moderate enhancement in 5 cases and obvious enhancement in 7 cases. There were 7 cases of high risk, 6 cases of middle risk and 7 cases of low risk.Conclusion:through analysis, CT can effectively diagnose stromal tumors originating from abdominal gastrointestinal tract and outside gastrointestinal tract, and can provide basis for differentiation of pathological grade and risk degree of tumors. It is a valuable diagnostic method for stromal tumors. Law.
