富血小板血浆引导骨再生在上颌美学区单牙种植修复中的应用

摘　　要：目的探讨在上颌美学区单牙种植修复治疗中应用富血小板血浆对引导骨再生的临床疗效。方法以山西省人民医院2015年4月至2017年6月诊治的上颌美学区内行单牙种植修复的86例患者作为此次研究对象,以随机数字表法分为观察组和对照组,每组43例。两组患者均接受上颌美学区内单牙种植修复治疗,其中观察组治疗期间应用混有富血小板血浆的引导骨再生技术,对照组引导骨再生并未应用富血小板血浆。结果观察组种植修复成功率为100.00%,对照组为97.67%,两组比较差异未见统计学意义(P>0.05);观察组骨吸收>1个螺纹者2例(4.65%),对照组为3例(6.98%),两组比较差异未见统计学意义(P>0.05);观察组患者在种植体修复即刻及修复后6、12个月的红色美学指数、白色美学指数评分均高于对照组(P<0.05)。主观满意度对比:两组色泽和咀嚼功能评分比较差异未见统计学意义(P>0.05);观察组附着高度和整体美观度均高于对照组(P均<0.05)。结论在上颌美学区的单牙种植修复中,通过应用含有富血小板血浆的引导骨再生技术治疗可取得满意效果,有利于维持种植修复的骨高度和宽度,能够达到较优的美学效果。Objective To investigate the clinical efficacy of platelet-rich plasma on bone regeneration in the single-tooth implant restoration in the maxillary aesthetic area. Methods A total of 86 patients who underwent single-tooth restoration in the maxillary aesthetic area from April 2015 to June 2017 in Shanxi Provincial People’s Hospital were enrolled in this study. The patients were randomly divided into observation group and control group, with 43 cases in each group. Both groups underwent single-tooth implant restoration in the maxillary aesthetic area. The patients in observation group were treated by platelet-rich plasma-guided bone regeneration during implanting, while patients in control group were given bone regeneration without platelet-rich plasma. Results The success rate of implant regeneration in observation group was 100.00%, and it was 97.67% in control group, however there was no significant difference between the two groups (P>0.05). There were two patients (4.65%) with bone resorption>1 thread in observation group and 3 patients (6.98%) with bone resorption >1 thread in control group, but there was no significant difference between the two groups (P>0.05). The pink esthetic score (PES) and white esthetic score (WES) of observation group were higher than those of control group immediately after implant restoration, 6 months and 12 months after implant restoration (P<0.05). The subjective satisfaction of the two groups was compared in the color and chewing function score, and there was no significant difference (P>0.05). The patients in observation group were significantly higher than those in control group in the satisfaction degree of attachment height and overall aesthetics (P<0.05). Conclusions In the single-tooth implant restoration in the maxillary aesthetic area, guided bone regeneration with platelet-rich plasma can achieve satisfactory results, which is conducive to the maintenance of bone height and width of implant restoration, and can achieve better aesthetic effects.
