B超引导下经皮穿刺引流在颌面颈部间隙感染的应用

摘　　要：目的探讨B超引导下经皮穿刺引流在颌面颈部间隙感染伴脓肿形成的临床应用。方法分析自2016年5月~2017年12月收治的50例颌面颈部间隙感染伴脓肿形成的患者。按手术方式不同,分为B超引导下经皮穿刺引流术组和传统脓肿切开引流术组各25例。前组应用B超引导下经皮穿刺引流技术,对深部脓腔冲洗引流,后组采用传统脓肿切开引流术,分析临床效果。结果两组患者在充分引流冲洗后,经积极抗感染均能治愈。经B超引导下穿刺引流术组术后并发症低于传统脓肿切开引流术组,差异有统计学意义(P<0.05)。结论B超引导下经皮穿刺引流技术具有微创,脓肿定位明确,手术创口小,可保护重要组织,术后并发症少,安全可靠,值得在临床推广应用。Objective To investigate the application of percutaneous puncture in maxillofacial and cervical space infection with the guide of B-mode ultrasound.Methods 50 patients infected in maxillofacial and cervical space with abscess and treated in our department from May 2016 to December 2017 were enrolled.According to the type of surgical treatment received,50 cases were divided into two groups:percutaneous puncture drainage group guided by B-mode ultrasound and traditional incision drainage group.The former group was treated with percutaneous puncture and drainage with the guide of B-mode ultrasound and the deep abscess cavity was drained.The latter group was treated with traditional abscess incision and drainage.The clinical effects were analyzed for both groups.Results Both groups were cured by active anti-infection treatment after adequate drainage.Statistical analysis shows the rate of post-operative complications of the former group was significantly lower than the latter group ( P <0.05).Conclusion Percutaneous puncture with the guide of B-mode ultrasound has less post-operative complications.In addition to its other merits like minimal invasion,definite abscess positioning,and small operative wound.It could protect the important tissue with safety and reliability,thus it is deserved to be applied in clinical practice broadly.
