摘 要:目的外泌体是一种能被大多数细胞分泌的细胞外微小膜泡,含有多种生物活性分子,如蛋白质、微小RNA(microRNA,miRNA)和长链非编码RNA(long-chain non-coding RNAs,lncRNAs),在肿瘤发生发展中起重要的作用。本研究探讨外泌体在非小细胞肺癌(non-small cell lung cancer,NSCLC)中的研究进展,更新外泌体在NSCLC中的理解。方法应用PubMed及中国知网数据库检索系统,以"外泌体、肺癌"为关键词,检索2005-2018年的相关文献,共检索到中文文献61篇,英文文献171篇。纳入标准:外泌体与肺癌相关性的科学研究。根据纳入标准分析文献47篇。结果外泌体作为细胞间质物质和信息交流的工具,通过不同机制促进肿瘤细胞的增殖、侵袭、迁移转移及血管形成。此外,外泌体能预测肿瘤对放化疗的敏感性和作为治疗过程中的疗效监测指标及药物递送的介质,还能进一步提高对NSCLC的诊断和预后的判断。结论 NSCLC相关的外泌体与肿瘤发生、放化疗抵抗和转移有关,还可以作为NSCLC诊断、预后、监测和治疗的新型生物标志物。OBJECTIVE Exosomes are extracellular microvesicles that are released from various cell and contain various nucleic acid molecules,such as proteins,microRNAs(miRNAs)and long-chain non-coding RNAs(lncRNAs),which play important roles in tumorigenesis.This review focused on the recent research on exosomes in NSCLC.METHODS PubMed and CNKI retrieval system were used to search for related literatures from 2005 to 2018,using"exosome,lung cancer"as the key words.A total of 61 Chinese literatures and 171 English articles were retrieved.Inclusion criteria:exosomes associated research in lung cancer.According to the inclusion criteria,45 articles were analyzed.RESULTS Exosomes,as an important medium for intercellular substance and information exchange,promote the proliferation,invasion,migration,metastasis and angiogenesis of tumor cells through complex mechanisms.In addition,exosomes also display a further diagnosis and prognosis value in non-small cell lung cancer,not only for its prediction of the radiotherapy and chemotherapy sensitivity,but also for its characteristic of drug delivery media and assessing therapeutic efficacy.CONCLUSIONS Non-small cell lung cancer-associated exosomes have been associated with tumorigenesis,radiochemotherapy resistance and metastasis.In clinic,exosomes can serve as novel biomarkers for the diagnosis,prognosis,monitoring and treatment of NSCLC.
