摘 要:目的 探讨脑梗死合并微出血与基质金属蛋白酶-9(MMP-9)基因分布异常的相关性。方法 纳入脑梗死病人158例,其中脑微出血(CMBs)组66例,非CMBs组92例。检测两组血糖、三酰甘油、总胆固醇、高密度脂蛋白、低密度脂蛋白、血清MMP-9水平;并取外周血标本提取DNA,采用聚合酶链反应-限制性内切酶片段长度多态性(PCR-RFLP)方法检测MMP-9基因启动子C1562T基因型,比较两组间MMP-9基因多态性频率分布的差异。结果 单因素分析显示,年龄、高血压、血清MMP-9水平与微出血密切相关(P<0.05)。CMBs组MMP-9基因启动子区CC基因频率及C等位基因明显高于非CMBs组(P<0.05)。CMBs组CC基因携带病人MMP-9水平较非CMBs组明显增高(P<0.001)。Logistic逐步回归分析显示,MMP-9基因启动子区C等位基因是脑微出血的独立危险因素[OR=2.570,95%CI(1.393,3.012),P=0.029]。结论 MMP-9基因启动子区C等位基因是急性脑梗死病人脑微出血的独立危险因素,病人脑微出血CC基因与MMP-9水平密切相关。Objective To investigate the correlation between cerebral infarction combined with microbleeds and the abnormal distribution of matrix metalloproteinase-9 (MMP-9) gene. Methods One hundred and fifty-eight cases of cerebral infarction were divided into cerebral microbleeds group( n =66) and non-cerebral microbleeds group( n =92).The blood glucose,triglycerides,total cholesterol,high density lipoprotein,low density lipoprotein and serum MMP-9 were checked.Peripheral blood samples of DNA extraction was collected.Polymerase chain and restriction fragment length polymorphism (PCR-RFLP) method was performed for detection of MMP-9 gene promoter C1562T gene type.The frequency distribution of MMP-9 gene polymorphism was compared. Results The univariate analysis showed that age,hypertension and serum MMP-9 level were closely related to cerebral microbleeds( P <0.05).The 1562 CC gene frequency and C allele in the promoter region of the MMP-9 gene were significantly higher than those in the control group.The level of MMP-9 in the patients with 1562 CC gene carrier increased significantly in cerebral microbleeds group.Logistic stepwise regression analysis showed that MMP-9 1562 C allele was an independent risk factor for cerebral microbleeds[OR=2.570,95% CI (1.393,3.012), P =0.029]. Conclusion the C allele in the promoter region of MMP-9 gene may be an independent risk factor for cerebral microbleeds in patients with acute cerebral infarction.C allele in patients with cerebral microbleeds may be closely related to MMP-9 level.
