减孔腹腔镜在老年直肠癌根治术中的应用效果及优势分析

摘　　要：目的探讨减孔腹腔镜在老年直肠癌根治术中的应用效果及优势。方法回顾性分析2014年10月至2018年3月期间老年直肠癌患者116例资料,分为多孔组和减孔组,每组58例,使用SPSS22.0软件包进行统计分析,近期疗效指标和疼痛程度评分用(x^-±s)表示,采用独立t检验;吻合口漏发生率、3年无病生存率、总生存率、复发率用,采用χ^2检验。P<0.05表示差异有统计学意义。结果两组患者的手术时间、吻合口漏发生率、术后首次排气时间以及术后3年的无病生存率、总生存率以及复发率比较,差异无统计学意义(P>0.05),在切口总长度、术中出血量以及疼痛程度评分的比较中,减孔组效果更佳,差异有统计学意义(P>0.05)。结论减孔腹腔镜应用在老年直肠癌根治术中,可显著降低患者的手术切口长度、减少出血量、减轻患者的疼痛,值得临床推广。Objective To explore the effect and advantage of reduced incision laparoscopic surgery in elderly patients undergoing radical resection of rectal cancer. Methods Retrospective analysis of 116 elderly cases of rectal cancer treated in our hospital from October 2014 to March 2018 was performed. It is divided into normal group and incision reduced group, 58 cases in each group. The SPSS22.0 software package was used for statistical analysis. The short-term efficacy and pain scores were expressed as (x^-±s), and the independent t test was used to compare. The anastomotic leakage rate, 3-year disease-free survival rate, overall survival rate, and recurrence rate were measured by χ^2 test. P<0.05 indicated that the difference was statistically significant. Results There was no significant difference in the operation time, the incidence of anastomotic leakage, the first time after operation, and the disease-free survival, overall survival rate, and recurrence rate between the two groups(P>0.05). Comparing the total incision length, intraoperative blood loss, and pain scores, the incision reduced group had better results(P>0.05).Conclusion Reduced incision laparoscopic surgery in the treatment of elderly patients of rectal cancer can significantly reduce the incision length, reduce the amount of bleeding, and relieve the pain of the patients. It is worthy of clinical promotion.
