不同术式治疗60例慢性泪囊炎的临床疗效比较

摘　　要：目的分析比较内窥镜下鼻腔泪囊吻合术及鼻泪管“Y”型支架植入术方式治疗慢性泪囊炎的疗效。方法对2017年1月至2018年6月在甘肃省人民医院眼科住院的慢性泪囊炎患者60例60只眼,30例行鼻泪管“Y”型支架植入术,为A组,30例行内窥镜下鼻腔泪囊吻合术,为B组,对比观察2组患者术后流泪症状,冲洗泪道判断泪道阻塞情况以及手术并发症,随访期为12个月。结果随访6个月,A组有效率为93.33%,B组有效率为96.67%,2组间比较,差异无统计学意义(P>0.05)。随访12个月时,A组有效率为90%,B组为93.33%(28/30),2组间比较,差异无统计学意义(P>0.05)。2组患者术后均无严重并发症发生。结论鼻泪管“Y”型支架植入术及内窥镜下鼻腔泪囊吻合术均具有创伤小,术后恢复时间短的特点,均能有效地治疗慢性泪囊炎。OBJECTIVE To analyze and compare the efficacy of endoscopic dacryocystorhinostomy and Y stent implantation in treatment of chronic dacryocystitis. METHODS Sixty patients with 60 eyes implicated were recruited from January 2017 to June 2018 in Gansu Provincial Hospital. Among them,30 patients (group A) underwent routine nasolacrimal Y stent implantation;the other 30 (group B) underwent endoscopic dacryocystostomy. The two groups were followed-up for 12 months,to observe the efficacy of two methods on chronic dacryocystitis. Tear secretion,lacrimal duct obstruction judged by lacrimal duct flushing and surgical complications were observed and compared. RESULTS After 6 months follow-up,the effective rate was 93.33% in group A and 96.67% in group B,and there was no statistical difference between the two groups (P>0.05). Twelve months after intervention,the effective rate of group A was 90%,and that of group B was 93.33%(28/30),and there was no statistically significant difference between the two groups (P>0.05). No serious complications occurred in the two groups. CONCLUSIONS The Y stent implantation and endoscopic dacryocystorhinostomy were equally effective in treatment of chronic dacryocystitis.
