坐骨神经旁注射丙泊酚对神经病理性疼痛大鼠痛阈的影响及机制研究

摘　　要：目的探讨坐骨神经旁注射丙泊酚对于神经病理性疼痛大鼠痛阈的影响及机制。方法 40只成年雄性Wistar大鼠采用随机数字量表法随机分为两组,坐骨神经结扎(CCI)模型组32只,假手术组8只;CCI模型组随机分为4组,每组8只,A组:CCI模型空白对照组,B组:丙泊酚组,C组:利多卡因组,D组:生理盐水组。观察大鼠疼痛反应、行为学变化及运动功能评分。免疫组化方法测定大鼠坐骨神经中NMDA-2B受体表达情况,对大鼠坐骨神经进行病理分级评分。结果在丙泊酚组及利多卡因注射组机械痛阈与热痛阈都有升高,与对照组相比差异有统计学意义(P<0.05)。注射治疗后大鼠运动功能评分丙泊酚组与对照组比较明显降低(P<0.05)。大鼠坐骨神经病理分级评分各组间无明显差异(P>0.05)。坐骨神经上NMDA-2B受体免疫组化检测丙泊酚组与对照组相比表达明显降低(P<0.05)。结论外周注射丙泊酚可能通过抑制外周神经中NMDA-2B受体的表达而起作用。Objective To investigate the effect of sciatic nerve injection of propofol in the CCI rat model, and to explore the mechanism. Methods Fourty male Wistar rats were divided randomly into two groups: CCI group (n=32) and sham group ( n = 8 );CCI group was divided randomly into four groups ( n = 8 ): group A : control group;group B : propofol group;group C: lidocaine group;group D: saline group. The behavior score, pain response and moving function evaluation were observed. Immunohistochemistry was used to detect the expression of NMDA-2B receptors in sciatic nerve of the rats. Results After treatment of propofol, the paw withdrawl mechanical threshold and paw withdrawl thermal latency of the chronic constriction injury to sciatic nerve rats in propofol group were higher than the control group ( P < 0. 05 ). The expression of NMDA?2B receptor on sciatic nerve of the rats in propofol group was lower than the saline and control group by immunohistochemical staining (P<0.05). The scores of sciatic nerves through electronic microscopy in each group had no statistic difference ( P > 0. 05). Conclusion Local injection of propofol beside sciatic nerve in neuropathic pain rats may participate by peripheral NMDA-2B receptors localized in the sciatic nerves.
