摘　　要：目的探讨颅内孤立性纤维性肿瘤/血管外皮瘤(solitary fibrous tumors/hemangiopericytoma,SFT/HPC)和血管瘤型脑膜瘤的临床表现、病理特征和免疫表型,为提高两种疾病的诊断及鉴别诊断提供相应的指导依据。方法回顾性分析SFT/HPC和血管瘤型脑膜瘤的各20例患者,应用HE染色及免疫组织化学染色方法进行病理学特征观察,结合影像学方法进行综合分析。结果二者均多见于中年人,但SFT/HPC具有一定的生物学侵袭性,而血管瘤型脑膜瘤呈良性生长方式;镜下SFT/HPC具有典型的“鹿角样”血管和爆炸样胶原,而血管瘤型脑膜瘤血管多不规则。结论结合患者临床表现、术前影像学、病理组织学,应用免疫组化(CD34,STAT6,EMA等),有助于提高颅内SFT/HPC和血管瘤型脑膜的鉴别诊断。Objective To analyze the clinical manifestation, pathological features, immunophenotype and to identify the differentiation of ( SFT/HPC ) and hemangiomatous meningioma. Methods Twenty patients with SFT/HPC and 20 with hemangiomatous meningioma were observed by HE staining and immunohistochemical staining, and combined with imaging methods for comprehensive analysis. Results Both of them were common in middle-aged people, but SFT/HPC had a certain biological invasiveness, while the hemangiomatous meningioma showed a benign growth pattern. Under the microscope, SFT/HPC had typical "antler" blood vessels and explosive collagen, while the blood vessels of hemangiomatous meningioma were more irregular. Conclusion Combined with the clinical manifestations, preoperative imaging, histopathology and immunohistochemical phenotype, immunohistochemistry ( CD34, STAT6, EMA, etc.) was helpful to improve the differential diagnosis of intracranial SFT/HPC and hemangiomatous meningioma.
