摘  要:目的探讨吲哚菁绿与亚甲蓝联合示踪在乳腺癌前哨淋巴结活检(sentinel lymph node biopsy,SLNB)中的临床应用价值。方法收集2017年1月~2018年6月本院收治的临床腋窝淋巴结阴性乳腺癌患者71例,均使用吲哚菁绿与亚甲蓝联合示踪前哨淋巴结(sentinel lymph node,SLN)。收集完整的病理以及病历资料。观察记录各组亚甲蓝与吲哚菁绿各自标记淋巴结的检出数、淋巴结检出率。结果吲哚菁绿检出70例患者共242枚淋巴结,亚甲蓝检出68例患者共128枚淋巴结。两者联合所有患者均被检出。20例患者检出SLN转移,共检出28枚阳性SLN。吲哚菁绿阳性SLN检出率为100%,而亚甲蓝阳性SLN检出率85%(P<0.05)。结论吲哚菁绿较亚甲蓝具有更高的SLN检出率以及检出数量,二者联合可显著提高SLN检出率与检出数量、降低假阴性率。Objective To explore the value of combination use of methylene blue(MB) and indole cyanine green fluorescence navigation for sentinel lymph node biopsy(SLNB) in breast cancer. Methods Seventy-one patients with clinical axillary lymph node negative breast cancer admitted to our hospital from January 2017 to June 2018 were collected, all of whom were combined with methylene blue and indole cyanine green to track sentinel lymph nodes(SLN). Complete pathology and medical records were collected. The number and detection rate of sentinel lymph nodes were observed and recorded in each group. Results Indocyanine green detected 242 lymph nodes in 70 patients, and methylene blue detected 128 lymph nodes in 68 patients. All patients were tested with a combination of the two dyes. A total of 28 positive lymph nodes were found in 20 patients. The positive lymph node detection ratio of indocyanine green to methylene blue was 100% VS 85%(P<0.05). Conclusion Indocyanine green has higher SLN detection rate and detection quantity than methylene blue, and the combination of the two can significantly improve SLN detection rate and detection quantity, and reduce false negative rate.
