摘 要:目的:分析左下第二磨牙舌侧管管长和倾斜角分别改变时,个性化舌侧矫治近中移动该牙的移动规律及牙周膜应力分布特征。方法:建立左下第一磨牙缺失的下颌舌侧矫治三维有限元模型,设置5 种工况近中牵引左下第二磨牙:工况一、二、三为舌侧管倾斜角为0°,管长各为3. 5、4. 0、4. 5 mm;工况四、五为舌侧管长为4. 0 mm,管倾斜角各为10°、20°。颊舌侧同时加载0. 74 N 力,分析该牙的初始位移趋势及牙周膜应力分布。结果:近中牵引左下第二磨牙时,相比工况一,工况二、三的舌向扭转及近中倾斜趋势减小,牙周膜应力减小;相比工况二,工况四、五的近中倾斜趋势减少、远中倾斜趋势增加,牙周膜应力值也减小。结论:相较于固定倾斜角并改变管长,固定舌侧管管长并改变倾斜角可使左下第二磨牙前移时近中倾斜趋势减少,整体前移趋势增加。Objective: To analyze the biomechanical characteristics during mandibular second molar mesialization when changing the length or the inclination angle of the lingual tube using customized lingual appliances and mini-implant in a 3D finite element model. Methods: 5 different loading methods( 1,2 and 3: the lingual tube length of 3. 5,4. 0 and 4. 5 mm and the inclination angle of 0° were respectively loaded;4 and 5: the lingual tube length of 4. 0 mm and the angle of 10° and 20° were loaded respectively. The force of 0. 74 N was loaded at both buccal and lingual sides). The initial displacements of the left lower second molar and stress distribution in the periodontal ligaments were analyzed. Results: The inclination of the molar was decreased when the angle was constant but the length increased,and the same condition was observed when the angle increased but the length was constant. The inclination was changed from the mesial inclination to distal inclination while the angle changed only. The periodontal ligaments stress distribution reduced under all 5 types of loading. Conclusion: Less mesial inclination potential of the left mandibular second molar may be induced when the lingual tube length is constant and the angle is changed than that when the length is changed and the angle is constant.
