摘 要:目的探讨新型快速病理诊断技术在经直肠超声(TRUS)阴性前列腺癌(PCa)穿刺中的临床价值.方法选择22例TRUS阴性PCa的患者,在前列腺穿刺过程中采用新型快速病理诊断技术对穿刺组织滚片进行细胞学初诊,依据快速病理诊断结果调整前列腺穿刺方案,分析新型快速病理诊断技术在TRUS阴性PCa穿刺中的临床价值.结果新型快速病理诊断技术开展所需空间小,无污染,诊断耗时约2~3min,不影响正常前列腺穿刺操作.可依据快速病理诊断结果调整前列腺穿刺方案,减少漏诊的发生;高度异型细胞可表现为核大深染,核浆比失调,伴有明显核仁的异型细胞更具诊断提示意义.结论TRUS阴性PCa穿刺时易发生漏诊,临床危害大.新型快速病理诊断技术可减少漏诊的发生,提高前列腺穿刺的首次活检阳性检出率,值得临床广泛推广实行.Objective To explore the clinical value of new rapid pathological diagnosis tech-nology in prostate cancer(PCa) biopsy with negative transrectal ultrasonography (TRUS) findings. Methods A total of 22 TRUS negative PCa patients were performed with a new rapid pathological di-agnosis technology during prostate biopsy procedure. According to the results of this technology, the biopsy approach was adjusted in prostate biopsy procedure. The clinical value of new rapid pathological diagnosis technology in PCa biopsy with negative TRUS findings was analyzed. Results The new rapid pathological diagnosis technology requires smaller space with no pollution. The whole diagnosis proce-dure takes about 2 to 3 minutes and does not affect the normal prostate biopsy procedure . The operator can adjust the prostate procedure in operation according to the results of this technique. Therefore, this may reduce missed diagnosis. Highly heteromorphic cells can be characterized by large nuclear stai-ning, dysregulation of nuclear and plasma. Additionally,the heteromorphic cells with distinct nucleolus are more diagnostic. Conclusions The TRUS negative PCa biopsy is prone to missed diagnosis and it has great clinical harm. The new rapid pathological diagnosis technology can reduce the occurrence of missed diagnosis and increase the PCa detection rate. This technique is worthy of wide clinical applica-tion.
