摘 要:目的探讨高频彩色多普勒超声和弹性成像对微小乳腺癌和乳腺良性结节的鉴别诊断价值。方法选择281例女性乳腺疾病患者,根据术后病理结果的不同将其分为微小乳腺癌患者79例和乳腺良性结节患者202例。全部患者均接受了高频彩色多普勒超声和弹性成像检查。比较微小乳腺癌与乳腺良性结节的钙化类型、乳腺影像报告和数据系统(BI-RADS)分级情况和弹性应变率(SR)比值。结果微小乳腺癌与乳腺良性结节钙化的发生率比较,差异无统计学意义(P﹥0.05),但钙化类型比较,差异有统计学意义(P﹤0.01)。微小乳腺癌的钙化类型以Ⅰ型多见,乳腺良性结节的钙化类型以Ⅲ型多见。微小乳腺癌与乳腺良性结节的BI-RADS分级情况比较,差异有统计学意义(P﹤0.01),其中,微小乳腺癌的BI-RADS分级以4~5级为主,乳腺良性结节的BIRADS分级以2~3级为主。微小乳腺癌的弹性SR比值高于乳腺良性结节(P﹤0.05)。结论微小乳腺癌的钙化类型主要为Ⅰ型,BI-RADS分级主要为4~5级,且弹性SR比值较高。高频彩色多普勒超声和超声弹性成像检查对微小乳腺癌和乳腺良性结节具有一定的鉴别诊断价值,值得临床推广应用。Objective To investigate the value of high-frequency color Doppler ultrasound and elastography in differential diagnosis of minimal breast cancer and benign breast nodules.Method A total of 281 patients with breast diseases were selected.According to the pathological results after operation,79 patients with minimal breast cancer and 202 patients with benign breast nodules were divided into two groups.All patients underwent high-frequency color Doppler ultrasound and elastography.The calcification type,breast imaging-reporting and data system(BI-RADS)classification and elastic strain rate(SR)ratio between minimal breast cancer and benign breast nodules were compared.Result There was no significant difference in the incidence of calcification between minimal breast cancer and benign breast nodules(P>0.05).There was significant difference about calcification types between minimal breast cancer and benign breast nodules(P<0.01).The most calcification type of minimal breast cancer was typeⅠ,while that of benign breast nodules was typeⅢ.There were significant difference between the BI-RADS classification of minimal breast cancer and benign breast nodules(P<0.01).Among them,minimal breast cancer were mainly of grade 4 and 5,and benign breast nodules were mainly of grade 2 and 3.The elastic SR ratio of minimal breast cancer was higher than that of benign breast nodules(P<0.05).Conclusion The calcification type of minimal breast cancer is mainly type I,the BI-RADS grade is grade 4 and 5,the elastic SR ratio is higher,which is significantly different from benign breast nodules.High-frequency color Doppler ultrasonography and elastography have certain value in differential diagnosis of minimal breast cancer and benign breast nodules,which is worthy of clinical application.
