摘  要:目的:探讨神经电生理联合超声检查在腕管综合征(CTS)及肘管综合征(CuTS)中的临床价值。方法:研究我院2018年1-11月收治的腕管综合征及肘管综合征患者共43例(57侧),均应用传统电生理检查,第二蚓状肌-骨间肌远端运动潜伏期差值法、短段传导时间测定,超声检査,分别比较传统神经电生理、联合电生理及超声检查的阳性率。结果:43例病例57侧传统神经电生理检査异常33侧(阳性率为58%),联合神经电生理检査组异常44侧(阳性率为77%),传统神经电生理联合超声检查组异常42侧(阳性率为74%),联合神经电生理及超声检查组异常52侧(阳性率为91%),单纯超声检查异常27侧(阳性率为47%)。发现卡压病因4例。结论:应用超声联合第二蚓状肌-骨间肌远端运动潜伏期差值法、短段传导时间测定法,可提高腕管、肘管综合征的诊断率,能明确卡压病变性质,值得临床应用。Objective: To investigate the applied clinical value of electrophysiology combined with ultrasonography in carpal tunnel and cubital tunnel syndrome. Method: From January 2018 to November 2018, 43 patients (57 limbs) with carpal tunnel syndrome(CTS) and cubital tunnel syndrome( CuTS) were treated with traditional electrophysiological method, second lumbrical and interossei distal motor latency difference (2LI-DML), short segment conduction time (SSCT) and ultrasonography. The positive rates in the patients were compared during the examination by traditional electrophysiology and electrophysiology combined with ultrasonography. Result: Out of 57 cases, 33 cases (58%) were abnormal in traditional electrophysiology examination, 44 (77%) abnormal in combined electrophysiology examination, 42 (74%) abnormal in traditional electrophysiology combined with ultrasound examination. 52 (91%) abnormal in combined electrophysiology and ultrasound examination, and 27 (47%) abnormal in single ultrasound examination. And the entrapment lesions were found in 4 cases. Conclusion: Traditional electrophysiology examination combined with 2LI-DML, SSCT and ultrasonography can increase the diagnostic rate of carpal tunnel and cubital tunnel syndromes. It can clarify the nature of entrapment lesions and is worthy of clinical application.
