固位纤维牙周夹板联合盐酸米诺环素软膏治疗重度牙周病患者的临床研究

摘　　要：目的探究固位纤维牙周夹板联合盐酸米诺环素软膏治疗重度牙周病患者的临床效果.方法选取我院86例重度牙周病患者,按照治疗方法分组,各43例.尼龙绳丝组予以尼龙绳丝结扎联合盐酸米诺环素软膏治疗,固位纤维组予以固位纤维牙周夹板联合盐酸米诺环素软膏治疗.对比两组治疗3个月后临床疗效、牙齿美观度、舒适度、治疗前后牙周袋深度(PD)、菌斑指数(PLI)评分、龈沟出血指数(SBI)评分、龈沟液肿瘤坏死因子-α(TNF-α)白介素-1β(IL-1β)、降钙素原(PCT)水平.结果固位纤维组治疗总有效率97.67%(42/43)较尼龙绳丝组79.07%(34/43)高(P<0.05);治疗后3个月固位纤维组PD、PLI评分、SBI评分较尼龙绳丝组低(P<0.05);治疗后3个月固位纤维组牙齿美观度、舒适度评分较尼龙绳丝组高(P<0.05);治疗后3个月固位纤维组龈沟液TNF-α、IL-1β、PCT水平较尼龙绳丝组低(P<0.05).结论与尼龙绳丝结扎相比,固位纤维牙周夹板联合盐酸米诺环素软膏治疗重度牙周病患者,疗效较高,可改善牙周组织状况,减轻炎症反应,且具有良好的美观度、舒适度.Objective To explore the clinical effect of retention fiber periodontal splint combined with minocycline hydrochloride ointment in the treatment of severe periodontal disease. Methods 86 patients with severe periodontal disease in our hospital were selected and divided into 43 cases each according to the treatment method. The nylon rope group was treated with nylon rope ligation combined with minocycline hydrochloride ointment, and the retention fiber group was treated with retention fiber periodontal splint combined with minocycline hydrochloride ointment. The clinical efficacy, dental aesthetics, comfort, periodontal pocket depth (PD), plaque index (PLI), gingival sulcus hemorrhage index (SBI), tumor necrosis factor-alpha (TNF-alpha), interleukin-1 beta (IL-1bet) and procalcitonin (PCT) levels were compared between the two groups after 3 months of treatment. Results The total effective rate of retention fiber group was 97.67%(42/43) higher than that of nylon wire group 79.07%(34/43)(P < 0.05);the PD, PLI and SBI scores of retention fiber group were lower than that of nylon wire group at 3 months after treatment (P < 0.05);the aesthetic and comfortable scores of teeth in retention fiber group were higher than that of nylon wire group at 3 months after treatment (P < 0.05);and the gingival sulcus of retention fiber group was higher than that of nylon wire group at 3 months after treatment (P < 0.05). The levels of TNF-a, IL- 1beta and PCT in liquid were lower than that in nylon rope group (P < 0.05). Conclusion Compared with nylon wire ligation, retention fiber periodontal splint combined with minocycline hydrochloride ointment is more effective in the treatment of severe periodontal disease. It can improve periodontal tissue condition, reduce inflammation, and has good aesthetic and comfort.
