摘 要:目的:探讨低频重复经颅磁刺激(rTMS)对脑梗死患者肢体运动功能障碍的康复作用。方法:选择本院2017年2月-2018年5月接诊的56例脑梗死患者,按随机数字表法分为常规药物组和联合治疗组,各28例。常规药物组给予抗血小板、活血化瘀和对症支持治疗,联合治疗组在常规药物组基础上配合低频重复经颅磁刺激。对两组临床疗效、干预前后神经功能缺损评分(NIHSS)、Berg平衡量表(BBS)评分、Fugl-Meyer评估(FMA)、Barthel指数(BI),患侧脑区运动诱发电位(MEP)潜伏期和波幅进行比较分析。结果:干预后联合治疗组治疗总有效率(92.86%)明显高于常规药物组(67.86%),差异有统计学意义(P<0.05);干预后,两组NIHSS、BBS、FMA和BI均优于干预前,联合治疗组NIHSS、BBS、FMA和BI均优于常规药物组(P<0.05)。两组干预后MEP潜伏期短于干预前,波幅高于干预前,联合治疗组MEP各项指标均优于常规药物组(P<0.05)。结论:低频重复经颅磁刺激对伴有肢体运动功能障碍的脑梗死患者疗效较好,能够显著改善NIHSS、BBS、FMA和BI等指标,对患者肢体运动功能的康复具有积极意义。Objective:To investigate the rehabilitation effect of low frequency repetitive transcranial magnetic stimulation(r TMS)on limb motor dysfunction in patients with cerebral infarction.Method:56 patients with cerebral infarction admitted to our hospital from February 2017 to May 2018 were divided into conventional drug group and combined treatment group according to the random number table method,28 cases in each group.The conventional drug group was given anti-platelet,activating blood circulation to remove blood stasis and symptomatic supportive treatment.The combined treatment group was treated with low-frequency rTMS on the basis of the conventional drug group.The clinical efficacy,neurofunction deficit score(NIHSS),berg balance scale(BBS)score,Fugl-Meyer assessment(FMA),barthel index(BI),latency and amplitude of motor evoked potential(MEP)of affected side brain were compared between the two groups.Result:The total effective rate of the combined treatment group was 92.86%significantly higher than that of the conventional drug group(67.86%),the difference was statistically significant(P<0.05).After intervention,the indexes of NIHSS,BBS,FMA and BI in two groups were better than those before intervention,and the NIHSS,BBS,FMA and BI of the combined treatment group were better than those of the conventional drug group(P<0.05).The MEP latency of the two groups was lower than that before intervention,and the MEP amplitude was higher than that before intervention,the indexes of MEP in the combined treatment group were better than those in the conventional drug group(P<0.05).Conclusion:Low-frequency rTMS is effective for patients with cerebral infarction with limb motor dysfunction.It can significantly improve NIHSS,BBS,FMA and BI,it has positive significance for the rehabilitation of limb motor function.
