摘 要:目的探讨彩色多普勒超声定位与定性诊断甲状腺良恶性结节的临床价值.方法以2017年1月—2018年12月为时限,择取该院102例甲状腺结节患者,依据穿刺活检或手术病理结果分为良性组(50例)和恶性组(52例),两组患者均行甲状腺彩色多普勒超声检查,对比观察两组甲状腺结节检出率以及定性诊断灵敏度与特异度.结果两组甲状腺结节检出率均为100.0%,恶性组定性准确率(96.15%)与良性组(96.0%)相当(x^2=0.856,P>0.05).恶性组结节形态欠规则86.54%,边缘毛糙90.38%,无包膜94.23%,灶内低回声82.69%,微小沙砾样钙化67.31%,血流Ⅱ级48.08%、Ⅲ级44.23%,各项指标与良性组均差异有统计学意义(x^2=9.314、12.735、10.536、11.034、8.644、6.810、7.435,P<0.05).结论彩色多普勒超声诊断鉴别良恶性甲状腺结节有效可行,值得临床推广.Objective To investigate the clinical value of color Doppler ultrasound in the localization and qualitative diagnosis of benign and malignant thyroid nodules. Methods From January 2017 to December 2018, 102 patients with thyroid nodules in our hospital were selected and divided into benign group (50 cases) and malignant group (52 cases) according to the results of puncture biopsy or surgical pathology. Both groups underwent thyroid color Doppler ultrasonography before operation. The detection rate of thyroid nodules, sensitivity and specificity of qualitative diagnosis were compared between the two groups. Results The detection rate of thyroid nodules in both groups was 100%. The qualitative accuracy of malignant group (96.15%) was similar to that of benign group (96.0%)(χ^2=0.856, P>0.05). In malignant group, the irregular regularity rate of nodules was 86.54%, the rate of marginal roughness was 90.38%, the rate of no capsule was 94.23%, the rate of focal hypoechoic was 82.69%, the rate of micro-gravel-like calcification was 67.31%, the rate of blood flow was grade Ⅱ 48.08%, the rate of grade Ⅲ was 44.23%, with statistical signifance (χ^2=9.314, 12.735, 10.536, 11.034, 8.644, 6.810, 7.435). Conclusion It is effective and feasible to differentiate benign and malignant thyroid nodules by color Doppler ultrasonography, which is worthy of clinical promotion.
