摘　　要：目的探讨单孔腹腔镜治疗巨大卵巢囊肿(直径≥10cm)的安全性和有效性。方法2016年12月~2018年12月,对27例巨大卵巢良性囊肿(囊肿直径10~15cm)行经脐单孔腹腔镜卵巢囊肿剥除术。脐部切口1.5~2cm,置入单孔port和腹腔镜、器械探查,将囊肿提至切口保护套下穿刺抽出囊液,剔除囊肿。结果27例均在单孔腹腔镜下顺利完成手术,手术时间40~65(51.3±1.2)min,术中出血量20~50ml,术后肛门排气时间6~18h,术后2~3d出院。无切口感染、皮下血肿、创面出血、器官损伤等并发症。结论对巨大卵巢囊肿行经脐单孔腹腔镜囊肿剥除术简单易行,安全可靠,疗效确切。Objective To observe the efficacy and safety of single-site laparoscopy for giant ovarian cysts.Methods Clinical data of 27 cases of giant ovarian cyst (10-15 cm in diameter) who received transumbilical single-site laparoscopic excision from December 2016 to December 2018 in our hospital were analyzed.The umbilical incision was 1.5-2 cm in length.The laparoscope and instruments were placed through the single port to explore the cyst.Then the cyst was pulled up under the incision protective sleeve for puncture to extract cyst fluid and then was removed.Results All the 27 cases of operations were successfully performed.The operation time was 40-65 min (mean,51.3±1.2 min).The intraoperative blood loss was 20-50 ml.The postoperative exhaust time was 6-18 h.The patients were discharged 2-3 days after surgery.No incision infection,subcutaneous hematoma,wound bleeding,organ damage or other complications were observed.Conclusion Single-site laparoscopic large ovarian cyst excision is simple,safe and reliable,with obvious curative effect.
