摘　　要：目的研究牙龈卟啉单胞菌(Pg)感染对慢性牙周炎患者外周血基质金属蛋白酶-3(MMP-3)和干扰素-γ(IFN-γ)水平影响。方法选取慢性牙周炎患者218例,轻度牙周炎113例、中重度牙周炎105例,同期选取115例健康人群作对照研究。观察研究对象Pg感染阳性率、阴性率和治疗前后血清MMP-3、IFN-γ水平。结果慢性牙周炎患者Pg感染阳性196例(89.91%),健康人群Pg感染阳性23例(20.00%),二者比较(P<0.05)。中重度牙周炎患者Pg阳性率高于轻度牙周炎患者,轻度牙周炎患者高于健康人群(P<0.05)。治疗前,轻、中重度慢性牙周炎患者Pg感染阳性率、MMP-3水平均高于健康人群、IFN-γ水平低于健康人群,中重度牙周炎患者Pg感染阳性率、MMP-3水平高于轻度牙周炎患者、IFN-γ水平低于轻度牙周炎患者(P<0.05)。治疗后较治疗前,轻、中重度牙周炎患者Pg感染阳性率、MMP-3水平下降、IFN-γ水平升高(P<0.05)。相关性分析显示Pg感染率与外周血MMP-3水平呈正相关关系,Pg感染率、MMP-3水平与IFN-γ呈负相关关系(P<0.05)。结论慢性牙周炎Pg感染与外周血MMP-3、IFN-γ具有相互促进效应,监测其水平,利于评价感染程度。OBJECTIVE To explore the influence of Porphyromonas gingivalis infection on levels of peripheral blood matrix metalloproteinase-3(MMP-3) and interferon-γ(IFN-γ) of patients with chronic periodontitis. METHODS Totally 218 patients with chronic periodontitis, 113 patients with mild periodontitis and 105 patients with moderate periodontitis were enrolled in the study, meanwhile, 115 healthy people were set as the control group. The positive rate and negative rate of P.gingivalis infection were analyzed, and the levels of serum MMP-3 and IFN-γ were observed before and after the treatment. RESULTS Totally 196(89.91%) patients with chronic periodontitis were positive for P.gingivalis infection, 23(20.00%) healthy people were positive for P.gingivalis infection, and there was significant difference(P<0.05). The positive rate of P.gingivalis of the patients with moderate periodontitis was significantly higher than that of the patients with mild periodontitis, and the positive rate of P.gingivalis of the patients with mild periodontitis was significantly higher than that of the healthy people(P<0.05). The positive rate of P.gingivalis infection and MMP-3 level of the patients with mild and moderate periodontitis were significantly higher than those of the healthy people before the treatment, while the IFN-γ level of the patients with mild and moderate periodontitis was significantly lower than that of the healthy people;the positive rate of P.gingivalis infection and MMP-3 level of the patients with moderate periodontitis were significantly higher than those of the patients with mild periodontitis, while the IFN-γ level of the patients with moderate periodontitis was significantly lower than that of the patients with mild periodontitis(P<0.05). The positive rate of P.gingivalis infection and MMP-3 level of the patients with mild and moderate periodontitis were lower after the treatment than before the treatment, while the IFN-γ level was higher after the treatment than before the treatment.
