128层CT容积扫描三维重建结合双期增强对早期周围型小肺癌的诊断价值

摘　　要：目的探讨128层CT容积扫描三维重建结合血管期和实质期动态增强对早期周围型小肺癌的诊断价值。方法选取2013年5月至2016年5月采用128层CT容积扫描并经手术病理证实的18例早期周围型小肺癌患者,均进行动态增强CT检查及图像三维重建处理,分析CT影像特征。结果诊断正确率达94.4%(17/18)。不同征象的显示率如下:①分叶征72.2%(13/18);②毛刺征50.0%(9/18);③软包硬磨玻璃结节征33.3%(6/18);④胸膜凹陷征77.8%(14/18);⑤血管集束征83.3%(15/18);⑥细支气管征27.8%(5/18);⑦典型恶性结节强化征83.3%(15/18)。结论 CT容积扫描三维重建结合双期增强能够全面、直观、准确地显示早期周围型小肺癌的恶性形态特征,通过分叶征、毛刺征、软包硬磨玻璃结节征、胸膜凹陷征、血管集束征、细支气管征对早期周围型小肺癌进行诊断。Objective To evaluate the diagnostic value of three-dimensional reconstruction and dual-phase enhancement of 128 slice CT volume scan in early-stage small lung cancer.Methods 18 cases of early-stage small lung cancer performed by 128 slice CT volume scan and clarified by pathology from May 2013 to May 2016 were collected. The dynamic contrast enhancement scans and three-dimensional reconstruction were performed in every case. CT features of lesions were analyzed. Results The rate of diagnostic accuracy was 94.4%(17/18).Different display rates of CT features were found,including lobulation in 72.2%(13/18),spiculation in 50.0%(9/18).hard ground-glass nodule with soft capsule in 33.3%(6/18),pleural indentation in 77.8%(14/18),blood vessel convergence sign in 83.3%(15/18),bronchiologram sign in 27.8%(5/18) and typical malignant nodules enhancement sign in 83.3%(15/18).Conclusion CT volume three-dimensional reconstruction combined with dual-phase enhancement can comprehensively, intuitively and accurately display the malighnant morphological features of early-stage small lung cancer, which is diagnosed from lobulation,spiculation,hard ground-glass nodule with soft capsule,pleural indentation,blood vessel convergence sign and bronchiologram sign.
