复方丹参滴丸联合曲安奈德治疗口腔黏膜下纤维性变的临床观察及对血清TGF-β1、IL-6的影响

摘　　要：目的:探究评价复方丹参滴丸联合曲安奈德治疗口腔黏膜下纤维性变(OSF)的临床观察及对血清TGF-β1、IL-6的影响情况。方法:选取口腔黏膜下纤维性变(OSF)患者167例,作为研究对象,将这些患者随机分为两组,对照组83例患者采用曲安奈德进行治疗,观察组84例患者在对照组的基础上联合服用复方滴丸进行治疗,观察比较两组患者治疗前后疼痛评分情况、黏膜病变区域面积、张口度、治疗效果以及中切全血黏度(MS)、血浆黏度(PV)、纤维蛋白原(FIB)、血清TGF-β1、IL-6的变化情况。结果:观察组患者治疗后疼痛评分(0.61±0.45)、黏膜病变区域面积(54.8±25.7)cm^2明显低于对照组疼痛评分(1.54±0.35)、黏膜病变区域面积(67.9±37.6)cm^2(P<0.05);治疗后张口度(3.65±1.31)mm、治愈率(94.05%)明显高于对照组张口度(2.93±1.25)mm、治愈率(78.31%)(P<0.05);治疗后观察组患者MS(5.21±0.38)mPa·s、PV(1.49±0.27)mPa·s、FIB(2.17±0.16)mPa·s明显低于对照组患者MS(7.65±0.65)mPa·s、PV(2.07±0.26)mPa·s、FIB(3.18±0.15)mPa·s,差异具有统计学意义(P<0.05);治疗后观察组患者的TGF-β1(6.47±3.12)μg/L、IL-6(7.47±3.24)pg/mL水平明显低于对照组患者TGF-β1(14.35±4.75)μg/L、IL-6(26.45±8.32)pg/mL(P<0.05)。结论:复方丹参滴丸联合曲安奈德可以有效缓解OSF患者的口腔灼痛感觉,减小黏膜病变区域面积,扩张开口度;有效改善血液黏度、血浆黏度和纤维蛋白原浓度;降低血清TGF-β1和IL-6含量,有效提高了临床治疗效果。Objective: To explore and evaluate the clinical effect of Compound Danshen Dropping Pills combined with triamcinolone acetonide on oral submucosal fibrosis(OSF) and serum TGF-β1 and IL-6. Methods: A total of 167 patients with OSF were randomly divided into two groups. Eighty-three patients in the control group were treated with triamcinolone acetonide. Eighty-four patients in the observation group were treated with Compound Dripping Pills on the basis of the control group. After treatment, the pain score, the area of mucosal lesion, mouth opening, therapeutic effect and the changes of whole blood viscosity(MS), plasma viscosity(PV), fibrinogen(FIB), serum TGF-β1 and IL-6 were observed and compared between the two groups. Results: The pain score(0.61±0.45) and the area of mucosal lesions(54.8±25.7) cm^2 in the observation group were significantly lower than those in the control group[(1.54±0.35),(67.9±37.6)cm^2](P<0.05). The mouth opening(3.65±1.31)mm and the cure rate(94.05%) were significantly higher than those in the control group. After treatment, MS(5.21±0.38)mPa·s, PV(1.49±0.27)mPa·s and FIB(2.17±0.16)mPa·s in the observation group were significantly lower than those in the control group [(7.65±0.65)mPa·s, PV(2.07±0.26)mPa·s, FIB(3.18±0.15)mPa·s]. After treatment, the levels of TGF-β1(6.47±3.12)μg/L and IL-6(7.47±3.24)pg/mL in the observation group were significantly lower than those in the control group[(14.35±4.75)μg/L,(26.45±8.32)pg/mL](P<0.05). Conclusion: Compound Danshen Dropping Pills combined with triamcinolone acetonide can effectively alleviate the burning sensation of oral cavity in OSF patients, reduce the area of mucosal lesion, expand the opening, improve the blood viscosity, plasma viscosity and fibrinogen concentration, reduce the levels of serum TGF-β1 and IL-6, and effectively improve the clinical efficacy.
