摘 要:目的:探讨首发青少年抑郁障碍(MDD)患者静息态脑磁图(rMEG)频谱特点。方法:对15例首发青少年MDD患者(MDD组)及14名正常对照者(对照组)进行rMEG扫描,比较两组频谱分布特点,分析MDD组频谱分布与汉密尔顿抑郁量表(HAMD)评分的关系。结果:①MDD组alpha频段能量左枕叶高于右侧,左颞叶低于右侧;theta频段能量左额、枕叶高于右侧;对照组alpha频段能量左枕叶高于右侧,beta频段能量左枕叶高于右侧,theta频段能量左额、颞及枕叶高于右侧,delta频段能量左颞叶高于右侧,左额叶低于右侧。②与对照组相比,MDD组alpha频段能量右中央区、颞及枕叶明显增高;beta频段能量双侧枕叶明显增高;theta频段能量右颞、枕叶明显增高;delta频段能量右颞、双顶叶明显增高;③MDD组HAMD总分与双侧颞、左顶及双侧枕叶delta频段能量呈正相关;HAMD焦虑因子与右枕叶theta频段能量及左顶、枕叶delta频段能量正相关,与左右顶叶alpha频段能量差呈正相关(P均<0.05)。结论:首发青少年MDD患者rMEG的alpha频段能量右侧过度激活,枕叶beta频段能量左侧优势减弱,右枕叶delta频段能量增强且与抑郁程度及焦虑症状相关。Objective: To investigate the characteristics of resting state magnetoencephalic (rMEG) spectrum in first-episode adolescent major depressive disorder (MDD). Method: Fifteen first-episode adolescent patients with MDD (MDD group) and 14 normal controls (control group) were scanned by rMEG.The characteristics of spectrum distribution were compared between the two groups.The relationship between spectrum distribution and Hamilton depression scale (HAMD) score was analyzed in MDD group. Results:①In the MDD group,the alpha band power was higher in left occipital region than right,and lower in left temporal region than right.The theta band power of regions in left frontal and occipital were higher than those in right. In the control group,alpha band power of left occipital region was higher than right, beta band power in left occipital region was higher than right,theta band power in regions of left frontal,temporal and occipital were higher than those in right, delta band power in left temporal region was higher than right,and left frontal region was lower than right.②Compared with the control group,the alpha band powers of the MDD group were significantly higher in regions of right central,temporal and occipital;beta band power in bilateral occipital region was increased;theta band powers were increased significantly in regions of right temporal and occipital;the delta band powers were significantly increased in region of right temporal and bilateral parietal.③The total HAMD score of MDD group was positively correlated with delta band powers of regions of bilateral temporal,left parietal and bilateral occipital.HAMD anxiety factor was positively correlated with the theta band power of regions of right occipital,left parietal,and the delta band power of left occipital region,and positively correlated with the alpha band power difference value of left parietal region (all P <0.05).
