老年甲状腺癌组织中MMP-2、TIMP-2及COX-2的表达及临床意义

摘　　要：目的探讨老年甲状腺癌组织中基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶组织抑制物-2(TIMP-2)、环氧化酶-2(COX-2)的表达及临床意义。方法应用免疫组织化学SP法检测海南省文昌市人民医院2015年6月至2017年2月手术切除的甲状腺癌组织(甲状腺癌组,n=58)、甲状腺瘤组织(甲状腺瘤组,n=46)和正常甲状腺组织(正常甲状腺组,n=38)中MMP-2、TIMP-2、COX-2的表达,分析MMP-2、TIMP-2、COX-2表达与甲状腺癌患者临床病理特征的关系,并采用Spearman等级相关分析甲状腺癌组织中MMP-2、TIMP-2、COX-2表达的相关性。结果甲状腺癌组MMP-2、TIMP-2、COX-2阳性表达率均明显高于甲状腺瘤组、正常甲状腺组,甲状腺瘤组MMP-2、TIMP-2、COX-2阳性表达率均明显高于正常甲状腺组(均P<0.05)。甲状腺癌组织中MMP-2、TIMP-2、COX-2阳性表达率与TNM分期、病理类型、是否存在淋巴结转移有关(均P<0.05),与性别、年龄、肿瘤直径无关(均P>0.05)。Spearman等级相关分析显示,甲状腺癌组织中MMP-2阳性表达率与TIMP-2、COX-2阳性表达率呈正相关(r_s=0.743、0.628,均P<0.01),TIMP-2阳性表达率与COX-2阳性表达率呈正相关(r_s=0.572,P<0.01)。结论老年甲状腺癌组织中存在MMP-2、TIMP-2、COX-2异常高表达,其表达水平与肿瘤TNM分期、病理类型、是否存在淋巴结转移有关,临床上可通过检测MMP-2、TIMP-2、COX-2表达情况来评估患者的病情和预后。Objective To investigate the expression of MMP-2,TIMP-2 and COX-2 and their correlation with clinical outcomes in elderly patients with thyroid carcinoma.Methods The expression levels of MMP-2,TIMP-2,and COX-2 were examined by immunohistochemistry in 58 thyroid cancer patients,46 thyroid adenoma patients,and 38 normal thyroid tissues.Results The expression levels of MMP-2,TIMP-2,and COX-2 in thyroid cancer were significantly higher than those in thyroid adenoma tissues and normal thyroid tissues.The high expression levels of MMP-2,TIMP-2,and COX-2 in thyroid cancer tissues were correlated with TNM staging,pathological classification and lymph node metastasis.Spearman’s correlation analysis indicated MMP-2 expression was directly correlated with TIMP-2 and COX-2(r_s=0.743,0.628,respectively,P<0.01).Similarly,TIMP-2 expression was directly correlated with COX-2 expression(r_s=0.572,P<0.01).Conclusion Combination detection of the expression of the MMP-2,TIMP-2,and COX-2 can provide a guidance for prognostic evaluation and risk classification of the elderly patients with thyroid carcinoma.
