摘  要:目的探讨3D打印技术结合计算机导航模拟技术在骨科住院医师培训中的效果。方法将住院医生随机分组,每组20人,两组用同一教师进行授课、演示。以腰椎骨折为示范病例,利用影像资料打印等比例3D模型。实验组在教师授课演示后,采用计算机导航辅助下对3D模型进行模拟椎弓根螺钉置入操作,每人6次;对照组采用徒手对3D模型进行椎弓根螺钉置入操作,每人6次。操作结束后对每人进行问卷调查与考核。结果实验组置钉准确度和置钉时间均优于对照组,差异有统计学意义(P <0.05)。教学方法评价实验组(94.2±5.4)分,对照组(86.3±6.9)分,差异有统计学意义(P <0.05)。教师评价实验组(93.5±6.0)分,对照组(90.4±4.5)分,差异无统计学意义(P> 0.05)。对课程感兴趣的百分比实验组90%,对照组50%,差异有统计学意义(P <0.05)。操作考核指标实验组(91.1±5.2)分,对照组为(84.6±7.2)分,差异有统计学意义(P <0.05)。结论培训评价和考核结果表明该方法提高了规培医生的学习兴趣,对课程反响较好。通过多次反复的练习让规培医生快速掌握指定操作,提高动手能力。计算机辅助导航让规培医生操作更规范、置钉时间更短、螺钉准确度更高。Objective To assess the effect of 3D printing technology combined with navigation assisted technology in orthopedic resident training. Methods The residents were randomly divided into two groups, with 20 in each group and both groups being taught by the same teacher. Isometric 3D models were printed using image data of lumbar vertebral fracture patients. All residents performed pedicle screws 6 times on one 3D model, with the ones in the test group assisted by computer navigation and the ones in the control group using only hands. An assessment and a survey with a questionnaire were conducted on each resident after training. Results The accuracy and time of nail placement in the experimental group were better than those in the control group, and the difference was statistically significant (P < 0.05). The score of teaching method evaluation was (94.2±5.4) in the experimental group and (86.3±6.9) in the control group. The difference was statistically significant (P < 0.05). There was no significant difference in the scores of teachers'evaluation between the experimental group (93.5±6.0) and the control group (90.4±4.5)(P > 0.05). The percentage of students interested in the course was 90% in the experimental group and 50% in the control group (P < 0.05). Operational assessment indicators experimental group (91.1±5.2) points, the control group (84.6±7.2) points, the difference was statistically significant (P < 0.05). Conclusion The training evaluation and assessment results show that this method can improve the interest of residents in the training and they can quickly acquire skills with repeated practice. The computer-assisted navigation can shorten the time for pedicle screws and increase the accuracy.
