体外反搏在脑梗死康复中的作用与机制研究

摘　　要：目的我们采用Barthel指数分析脑梗死患者神经功能恢复情况,利用磁共振灌注成像技术检查局部病灶处大脑血流量的变化,旨在探讨体外反搏在脑梗死康复中的作用及相关机制。方法选取我院神经内科120例脑梗死患者,发病5~7 d后随机分为两组,一组常规治疗为对照组;另一组常规治疗基础上加用体外反搏治疗,然后测取Barthel指数分析,脑血流灌注相关参数进行统计学分析。结果两组在采用不同方法治疗后,观察Barthel指数评分及磁共振脑血流相关指数,可见体外反搏组评分指数显著高于常规治疗组(P <0. 01)。脑血流参数MTT、CBF显著高于常规治疗组(P <0. 01),治疗后临床治愈率、显效率显著高于常规治疗组(P <0. 01)。结论本实验体外反搏的疗效明确,有利于脑梗死患者功能恢复,为脑梗死康复治疗提供了新的方法。Objective In order to study the effect and mechanism research of external counterpulsation in the rehabilitation of cerebral infarction,we analyzed neural functional recovery in patients with cerebral infarction using Barthel index,checked local lesions in the brain blood flow changes using magnetic resonance perfusion imaging technology. Methods On hundred and twenty patients with cerebral infarction were selected and were randomly divided into 2 groups a week later,the conventional treatment is the control group,another group received treatment of external counter pulsation on the basis of conventional treatment,Barthel index and cerebral blood flow perfusion parameters were analyzed by statistics.Results Our results showed that the barthel index score from the group of external counter pulsation was significantly higher than that from the conventional treatment group( P < 0. 01),cerebral blood flow parameters determined by MTT,CBF were found significantly higher than the conventional treatment group( P < 0. 01),the clinical cure rate showed that the efficiency was significantly higher than the conventional treatment group( P < 0. 01). Conclusion The curative effects of external counterpulsation is clear and advantageous in cerebral infarction patients functional recovery,and provides a new way for rehabilitation in this experiment.
