摘 要:目的探索可能影响不伴有继发性干燥综合征(sSS)的系统性红斑狼疮(SLE)患者发生干眼的细胞因子,并分析有关细胞因子的浓度与临床干眼指标之间的相关性。方法 54例诊断为SLE且符合纳入标准的患者被作为研究对象,根据患者的临床干眼指标分为干眼组( n =11)、干眼倾向组( n =25)、对照组( n =18)。用Western blot法检测各组患者血清中的基质金属蛋白酶2(MMP-2)及激肽释放酶13(Klk13)的含量,通过 ELISA法测量血清中的白介素1(IL-1)、 IL-6、肿瘤坏死因子α(TNF-α)的浓度。结果 MMP-2的蛋白相对表达量在干眼组与对照组之间的差异有统计学意义( P <0.05),IL-1的浓度在干眼倾向组明显高于对照组,差异有统计学意义( P <0.01),TNF-α的蛋白水平在干眼组与对照组之间的差异有统计学意义( P <0.05)。Klk13、IL-6的蛋白表达量在3组之间的差异无统计学意义( P >0.05)。结论 MMP-2、IL-1和TNF-α影响不伴有sSS的SLE患者的眼表情况,并且TNF-α可作为一个有效血清学指标反映眼表状况。Objective To investigate the cytokines which may influence the condition of the ocular surface in systemic lupus erythematosus (SLE) patients without secondary Sj gren s syndrome (sSS), and to analyze the correlation between cytokine concentrations and clinical dry eye parameters. Methods Fifty four patients who were diagnosed as SLE and met the study criteria were included in the study. They were divided into dry eye group ( n =11), dry eye tendency group ( n =25) and control group( n =18) according to their clinical dry eye parameters. The protein levels of matrix metalloproteinase 2 (MMP-2) and kallikrein 13(Klk13) in serum of each group were measured by Western blot. Serum concentrations of interleukin-1 (IL-1), interleukin-6 (IL-6) and tumor necrosis factor alpha (TNF-α) were evaluated by ELISA. Results The relative protein expression of MMP-2 showed significant difference between dry eye group and control group( P <0.05), the concentration of IL-1 was statistically different between dry eye tendency group and control group ( P <0.05), and for the protein concentration of TNF-α, there was significant difference between the dry eye group and control group ( P <0.05). The protein levels of Klk13 and IL-6 were not statistically different( P >0.05) in the three groups. Conclusion MMP-2, IL-1 and TNF-α influence the condition of the ocular surface in SLE patients without sSS and TNF-α can be used as an effectively serological indicator to reflect the condition of the ocular surface.
