益气生血法治疗肛瘘术后切口愈合缓慢的机理探讨

摘　　要：[目的]探讨益气生血法治疗肛瘘术后切口愈合缓慢的理论依据。[方法]分析中医古籍中对于肛瘘及肛瘘术后切口愈合缓慢的记载,结合临床归纳肛瘘术后切口愈合缓慢的新诊断标准,阐述气血与肛瘘术后创面的关系,总结益气生血法对于肛瘘术后切口愈合缓慢的作用。[结果]肛瘘术后愈合缓慢的创面属于中医溃疡的范畴,诊断时间多在三周以上,属于中医疮疡愈合的后期,病机主要为气血亏虚,失于濡养,致创面新肉生长缓慢,迁延不愈。针对其病因病机,采用益气生血的治疗原则,并选用益气生血法代表方当归补血汤治疗,所用方药不仅切中肛瘘术后切口愈合缓慢的病机关键,又符合现代药理学研究结果,可促进术后创面的恢复。[结论]运用补气生血法治疗肛瘘术后切口愈合缓慢有深厚的理论基础,值得临床参考、借鉴和推广。[Objective] To explore the theoretical basis of the treatment of slow healing of anal fistula by benefiting Qi for promoting production of blood.[Method] This paper analyzes ancient Chinese books which are associated with the slow incision healing after anal fistula and generalizes the new diagnostic criteria for the slow incision healing after anal fistula combined with clinical experience.We summarize the relationship among Qi,blood and anal fistula wounds.What is more,the effect of benefiting Qi for promoting production of blood method on slow incision healing after anal fistula is also demonstrated.[Result] The wounds with slow healing after anal fistula belong to the category of ulcers in traditional Chinese medicine.The diagnosis time is far beyond three weeks.And then it also belongs to the latest stage of healing sores in traditional Chinese medicine.The main pathogenesis of this disorder is the deficiency of Qi and blood,which results in regenerated tissue growing tardily and prolonging the healing time.In view of its etiology and pathogenesis,the principle of benefiting Qi for promoting production of blood method is adopted,and its representative prescription "Danggui Buxue decoction"is used in the treatment of this disease.This prescription is not only corresponding to the pathology of the slow incision healing after anal fistula,but also conforms to modern pharmacological research.As a result,recovery of postoperative wounds can be promoted.[Conclusion] The application of benefiting Qi for promoting production of blood method for the treatment of anal fistula has a rich theoretical basis for the slow incision healing after anal fistula operation.It is worthy being learned and popularized in clinical practice.
