反流性食管炎患者食管pH值的影响因素分析

摘　　要：目的探讨反流性食管炎(RE)患者食管pH值的影响因素.方法选择晋煤集团总医院2017年6月至2018年6月收治的35例食管pH值监测正常的RE患者作为观察组,另选同期40例食管pH值监测异常的RE患者作为对照组,分析影响RE患者食管pH值的因素.结果观察组RE分级比较差异未见统计学意义(P>0.05);观察组与对照组幽门螺杆菌感染率比较差异未见统计学意义(P>0.05);观察组合并裂孔疝的发病率为0.00%,对照组合并裂孔疝的发病率为20.00%,两组比较差异有统计学意义(P<0.05);观察组患者的肌静息压力高于对照组,两组比较差异有统计学意义(P<0.05),但两组下食管括约肌松弛率比较差异未见统计学意义(P>0.05);观察组患者食管体部肛侧蠕动高于对照组,差异有统计学意义(P<0.05);其他如中下段、中上段、口侧蠕动及峰-峰速度、波-波速度等指标比较,两组比较差异未见统计学意义(P>0.05).结论幽门螺杆菌感染对RE患者食管pH值影响并不明显,而较强的屏障功能以及清除功能则可能是影响RE患者pH值的重要原因.Objective To explore the influencing factors of esophageal pH in patients with reflux esophagitis(RE).Methods Thirty-five patients with RE who had normal esophageal pH value in General Hospital of Jinmei Group from June 2017 to June 2018 were selected as the observation group.Another 40 patients with abnormal esophageal pH value were selected as the control group.The cause of abnormal esophageal pH in some patients with RE was analyzed.Results There was no significant difference in RE classification in the observation group(P>0.05).There was no significant difference in the Helicobacter pylori infection rate between the two groups(P>0.05).The incidence of combined and hiatal hernia was 0.00%,the incidence of combined and hiatal hernia was 20.00%,and the difference between the groups was significant(P<0.05).The lower esophageal sphincter relaxation(LESP)of the observation group was significantly higher than that of the control group,and the difference between the groups was significant(P>0.05),but there was no significant difference in LESR(P>0.05).The anal side peristalsis of the esophageal body in the observation group was significantly higher than that of the control group,and the difference between the groups was significant(P<0.05).There were no significant differences in other indexes such as the middle and lower segment,the middle and upper segment,the orbital peristalsis and the peak-to-peak velocity,wave-wave velocity,between the two groups.(P>0.05).Conclusions Helicobacter pylori infection has no significant effect on the esophageal pH of patients with RE,and strong barrier function and clearance function may be important factors affecting the normal pH value of RE patients.
