不同厚度颏部安氏Ⅱ~1类患者颏部形态的变化研究

摘　　要：目的:对安氏Ⅱ~1女性错牙合畸形患者正畸治疗前后颏部软组织形态的变化进行比较,讨论颏部软组织本身的厚度对颏部软组织形态治疗前后变化的影响。方法:选出在新疆医科大学第五附属医院口腔正畸科就医的100例女性患者,年龄18~35岁。每位患者拍摄头颅侧位片,对颏部软组织厚度进行测量,并根据测量结果进行分组建立颏厚度分类标准。选择50例乌鲁木齐地区安氏Ⅱ~1女性患者,均拍摄治疗前后的头颅侧位片,测量治疗前的颏部软组织厚度,根据第一部分测量结果进行分组:薄颏型组、均颏型组和厚颏型组,观察并研究不同厚度颏部形态在治疗前后的变化。结果:①治疗前三组颏部软组织测量项目的比较中,薄颏型组的颏唇沟深度明显小于均衡型组和厚颏型组;薄颏型组的颏唇角则大于厚颏型组与均衡型组;薄颏型组的颏沟倾角小于厚颏型组,与均衡型组无明显差别;②三组治疗结束后自身对比结果显示:颏部软组织厚度均减小,颏唇沟深度减小,颏唇角、颏沟倾角在薄颏型组治疗前后的变化是减小,而在其他两组中的变化是变大;③治疗结束后薄颏型组的颏厚度与其他两组相比是薄的,差异具有统计学意义(P<0.05);薄颏型组的唇沟深度小于厚颏型组,三组中其他测量指标之间也有区别,但均无统计学差异(P>0.05)。结论:不同厚度的颏部软组织的形态之间存在差异,治疗前后薄颏型的变化较理想;均衡型、厚颏型颏部应更加注意前牙内收量等因素。Objective To compare the changes of mental soft tissue morphology of Angle Ⅱ^1 female malocclusion patients before and after orthodontic treatment, and to discuss the influence of the thickness of mental soft tissue itself on the changes of mental soft tissue morphology before and after treatment. Methods 100 female patients aged between 18 and 35 were selected from the Department of Orthodontics, Fifth Affiliated Hospital of Xinjiang Medical University. Each patient took lateral skull film to measure the thickness of the mental soft tissue, and according to the measurement results, the classification standard of the mental thickness was established. Fifty female patients with Angle Ⅱ~1 were selected from the Fifth Affiliated Hospital of Xinjiang Medical University. Lateral cephalometric films were taken before and after treatment. The thickness of mental soft tissue before treatment was measured. According to the results of the first part, the patients were divided into three groups: thin chin group, balanced group and thick chin group. Results ①In the comparison of mental soft tissue measurement items among the three groups before treatment, the chin-lip groove depth of the thin chin group before treatment were significantly smaller than those of the balanced group and the thick chin group. Chin-lip angle of the thin chin group before treatment were significantly greater than other two group. the chin groove inclination angle of the thin chin group was smaller than that of the thick chin group, and there was no significant difference between the thin chin group and the even chin group.②The self-comparison results of the three groups after treatment showed that the thickness of mental soft tissue decreased, the depth of chin-lip sulcus decreased, and the change of chin-lip angle and chin-lip inclination decreased before and after treatment in the thin-chin group, while the change in the other two groups was larger.③After tr
