摘　　要：目的:探讨使用Embosphere微球与PVA颗粒栓塞子宫动脉治疗子宫肌瘤的临床疗效及并发症的情况。方法:回顾性分析明确诊断的子宫肌瘤患者49例,其中23例使用Embosphere微球栓塞,26例使用PVA颗粒栓塞。观察两组患者子宫肌瘤大小、血红蛋白、痛经程度、术后疼痛、胰岛素样生长因子-I及受体的改变。结果:与术前比较,术后两组在子宫肌瘤大小、血红蛋白、胰岛素样生长因子-I及受体、痛经缓解程度、术后疼痛评分方面均有改变。两组术后中度痛经缓解率、术后24 h疼痛评分、胰岛素样生长因子-I及受体变化方面比较,差异具有统计学意义( P <0.05)。结论:使用Embosphere微球与PVA颗粒栓塞子宫动脉治疗子宫肌瘤对改善宫肌瘤大小、升高血红蛋白、缓解痛经程度具有较好的疗效。在临床疗效相似的情况下,使用Embosphere微球在术后可减轻疼痛,同时降低胰岛素样生长因子-I及受体水平。Objective: To investigate the clinical efficacy and complications of uterine artery embolization with Embosphere microspheres and PVA particles in the treatment of uterine myoma. Methods: A retrospective analysis was made in 49 patients diagnosed with uterine leiomyoma,23 cases using Embosphere microsphere embolization and 26 cases with PVA particle embolization.The changes of the size of uterine myoma,hemoglobin,dysmenorrhea,postoperative pain,insulin-like growth factor-I(IGF-I) and receptor(IGF-IR) were observed. Results: Compared with preoperative,the uterine fibroids size,hemoglobin,IGF-I and IGF-IR,dysmenorrhea relieving degree and pain score were changed in both groups after operation.There were significant differences in remission rate of moderate dysmenorrhea,24 hours postoperative pain score,IGF-I and IGF-IR changes between the two groups( P <0.05). Conclusion: Embolization of uterine artery with Embosphere microspheres and PVA particles can improve the size of uterine leiomyoma,increase hemoglobin and relieve dysmenorrhea.In the case of similar clinical efficacy,the use of Embosphere microspheres can reduce pain and decrease the level of IGF-I and IGF-IR.
