紫杉醇腹腔热灌注配合常规化疗对晚期胃癌腹腔转移的应用价值

摘　　要：目的探析紫杉醇腹腔热灌注配合常规化疗在晚期胃癌腹腔转移中的治疗效果。方法方便选取2017年1月—2019年1月该院肿瘤科接诊的40例晚期胃癌腹腔转移患者,采用数字随机法将其分为两组,各20例。对照组给予常规化疗,研究组在此基础上配合紫杉醇腹腔热灌注,比较治疗效果。结果研究组疾病控制率80.0%高于对照组50.0%,癌胚抗原CEA(12.2±3.4)ng/mL、糖类抗原CA125(33.8±10.2)U/mL低于对照组(13.9±2.9)ng/mL、(38.9±7.2)U/mL,糖类抗原CA199(31.6±10.1)U/mL高于对照组(26.3±9.4)U/mL,差异有统计学意义(χ^2=3.956,P=0.047;t=1.701,P=0.049;t=1.827,P=0.038;t=1.718,P=0.047);药物不良反应发生率15.0%低于对照组35.0%,差异无统计学意义(χ^2=2.133,P=0.144)。结论晚期胃癌腹腔转移患者选择紫杉醇腹腔热灌注配合常规化疗安全可靠,还能改善肿瘤标记物指标,值得推广应用。Objective To investigate the therapeutic effect of paclitaxel peritoneal hyperthermic perfusion combined with conventional chemotherapy in the treatment of advanced gastric cancer with abdominal cavity metastasis.Methods Convenient forty patients with advanced gastric cancer who underwent abdominal metastases from January 2017 to January 2019 were enrolled in this study.They were divided into two groups,20 cases each.The control group was given conventional chemotherapy,and the study group was combined with paclitaxel peritoneal hyperthermia to compare the therapeutic effects.Results The disease control rate of the study group was 80.0%higher than that of the control group,which was 50.0%.The carcinoembryonic antigen CEA(12.2±3.4)ng/mL and the carbohydrate antigen CA125(33.8±10.2)U/mL were lower than the control group(13.9±2.9)ng/mL,(38.9±7.2)U/mL,carbohydrate antigen CA199(31.6±10.1)U/mL was higher than the control group(26.3±9.4)U/mL,the difference was significant(χ^2=3.956,P=0.047;t=1.701,P=0.049;t=1.827,P=0.038;t=1.718,P=0.047);the incidence of drug adverse reactions was 15.0%lower than that of the control group 35.0%,but there was no significant difference(χ^2=2.133,P=0.144).Conclusion The patients with advanced gastric cancer with peritoneal metastasis who choose paclitaxel peritoneal hyperthermia combined with conventional chemotherapy are safe and reliable,and can improve the tumor marker index,which is worthy of popularization and application.
