摘  要:目的探讨脑梗死患者的动脉粥样硬化程度和载脂蛋白A5(ApoA5)-1131T/C 和553G/T 2 位点的关系。方法随机选取2015 年1 月—2017 年1 月陕西省人民医院收治的脑梗死患者120 例进行临床研究。所有患者给予颈动脉血管超声检查、基因DNA 提取及血脂相关指标检查,比较-1131T/C 位点和553G/T 位点间的颈总动脉内膜-中层厚度(CCA-IMT)和颈内动脉内膜-中层厚度(ICA-IMT)之间的差异,比较-1131T/C 位点和553G/T 位点不同基因型的总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)水平,比较常见单倍型配对间的CCA-IMT 和ICA-IMT 水平。结果-1131T/C位点基因型为CC 的患者与基因型为TT+TC 的患者CCA-IMT 比较差异无统计学意义(P >0.05),-1131T/C位点基因型为CC 的患者与基因型为TT+TC 的患者ICA-IMT 比较差异有统计学意义(P <0.05),CC 的ICAIMT高于TT+TC;553G/T 位点基因型GG 与TT+GT 基因型的CCA-IMT 与ICA-IMT 比较差异无统计学意义(P >0.05);-1131T/C 位点基因型CC 与TT+TC 位点TC、HDL-C、LDL-C 水平比较差异无统计学意义(P >0.05),-1131T/C 位点基因型CC 与TT+TC 位点TG 水平比较差异有统计学意义(P <0.05),CC 位点TG 水平高于TT+TC 水平;553G/T 位点基因型GG 与TT+GT 位点患者TC、TG、HDL-C 及LDL-C 水平比较差异无统计学意义(P >0.05),TG-TG、TG-CG、TG-CT、CG-CG 的CCA-IMT 比较差异无统计学意义(P >0.05),而ICA-IMT 比较差异有统计学意义(P <0.05),其中CG-CG 型最高,TG-CG 型最低。结论脑梗死患者载脂蛋白A5 基因-1131T/C 位点与ICA-IMT、TG 的水平差异相关,不同单倍型配对的患者ICA-IMT 水平不同。Objective To observe the relationship between the degree of atherosclerosis and apolipoprotein A5 (ApoA5)- 1131T/C and 553G/T in patients with cerebral infarction. Methods A total of 120 patients with cerebral infarction who were admitted in our hospital from January 2015 to January 2017 were involved in this study. Carotid artery ultrasound, gene DNA extraction and blood lipid related indicators were recorded. The difference of CCAIMT and ICA-IMT between - 1131T/C and 553G/T loci were observed. The levels of TC, TG, HDL-C and LDL-C of different genotypes at - 1131T/C and 553G/T loci were obtained. CCA-IMT and ICA-IMT levels were compared between common haplotype pairs. Results There was no significant difference in CCA- IMT (carotid intimamedia thickness) between patients with CC genotype and TT+TC genotype at -1131T/C locus (P > 0.05). However,ICA-IMT was higher in CC type at -1131T/C locus when compared with that in TT+TC (P < 0.05). There was no significant difference in CCA-IMT and ICA-IMT of GG and TT+GT genotype at 553G/T locus (P > 0.05). No obvious difference in levels of TC, HDL-C, LDL-C and TG was observed between CC genotype and TT+TC genotype at -1131T/C locus (P > 0.05), while the levels of TG at CC locus were higher than that at TT+TC locus (P < 0.05). The levels of TC, HDL-C, LDL-C and TG at 553G/T locus were comparably similar between CC genotype and TT+GT genotype at -1131T/C locus locus (P > 0.05). There were no statistical differences in CCAIMT between TC, HDL-C, LDL-C, TG-CG, TG-CT, and CG-CG (P > 0.05), while ICA-IMT was the highest in CG-CG type while the lowest in TG-CG type. Conclusion Apolipoprotein A5 gene-1131T/C locus is correlated to levels of ICA-IMT and TG in patients with cerebral infarction, which may interfere the atherosclerosis degree.
