摘 要:目的:探讨超声剪切波弹性成像对脑卒中腓肠肌硬度变化的定量指标价值。方法:选取2019年1月~2019年3月在温州医科大学附属第一医院康复科住院的脑卒中单侧偏瘫患者20例作为卒中组,健康志愿者20例为对照组,对卒中组患者进行跖屈肌的临床痉挛指数、改良Ashworth痉挛量表评定;利用超声剪切波弹性成像技术分别测量对照组和卒中组健侧、患侧腓肠肌内侧头在不同踝关节角度的杨氏模量平均值,比较杨氏模量与病程、肌痉挛的相关性。结果:患侧杨氏模量在休息位和背屈位大于健侧和对照组(P<0.05);腓肠肌被动伸展时杨氏模量逐渐增大,且患侧的变化大于健侧(P<0.05);患侧跖屈位杨氏模量与病程相关系数为0.674,呈强相关(P<0.05);与临床痉挛指数的相关系数为0.526,呈中等相关(P<0.05)。结论:超声剪切波弹性成像可作为分析卒中后肌肉结构功能变化的定量指标。Objective: To explore the value of ultrasonic shear wave elastography in quantitative index of gastrocnemius muscle stiffness changes for patients with stroke. Methods: Patients with unilateral hemiplegia after stroke who were hospitalized in the rehabilitation department of No.1 Affiliated Hospital of Wenzhou Medical University from January 2019 to March 2019 were selected. 20 patients were included in the stroke group and 20 healthy volunteers were included in the control group. The clinical spasm index and modified Ashworth spasm scale of the metatarsals flexor in the stroke group were assessed. The average Young's modulus of medial head of gastrocnemius muscle on the healthy side and the affected side at different ankle joint angles in the control group and the stroke group were measured by ultrasonic shear wave elastography. The correlation between Young's modulus and course of disease, muscle spasm were compared. Results: Young's modulus in resting position and dorsal flexion position of affected side was higher than that of healthy side and control group ( P <0.05);Young's modulus increased gradually when gastrocnemius was stretched passively, and the change of affected side was greater than that of healthy side ( P <0.05);The correlation coefficient between Young's modulus in metatarsals flexor position and disease course was 0.674, with a strong correlation ( P <0.05);And the correlation coefficient with clinical spasm index was 0.526, with a moderately correlation ( P <0.05). Conclusion: Ultrasound shear wave elastography could be used as a quantitative index to analyze the changes of muscle structure and function for patients after stroke.
