前列地尔对老年脑血栓VBI的血液流变学指标、颈动脉斑块及动脉硬化指数的影响

摘　　要：目的:研究以前列地尔为主的治疗方案对老年血栓椎基底动脉供血不足(VBI)血液流变学指标、颈动脉斑块及动脉硬化指数(ASI)的影响。方法:选取80例老年脑血栓VBI患者,随机分为观察组与对照组,每组40例。对照组在基础治疗基础上给予丹参川穹嗪,观察组在对照组的基础上给予前列地尔用药,两组连续治疗14 d。比较两组临床有效率、血流动力学、血液流变学指标、动脉粥样硬化指数、听觉脑干诱发电位(ABR)的波潜伏期及波间期结果。结果:治疗14 d后,观察组临床总有效率(92.50%)较对照组(70.00%)高(P<0.05)。观察组左椎动脉血流速度(Vm)、右椎动脉Vm、基底动脉Vm显著高于对照组,血浆比黏度、红细胞聚集指数、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)和ASI显著低于对照组(均P<0.05)。观察组SF-36生活质量简表中躯体角色功能(RP)、总体健康(GH)、社会功能(SF)、情绪角色功能(RE)、心理健康(MCS)评分均显著高于对照组,波Ⅲ、V潜伏期、波Ⅰ~Ⅲ期、波Ⅲ~Ⅴ期、波Ⅰ~Ⅴ期显著短于对照组(均P<0.05)。结论:以前列地尔为主的治疗方案用于治疗老年脑血栓VBI疗效显著,能有效改善患者血液流变学指标,稳定颈动脉斑块,降低ASI。Objective:To investigate the effect of alprostadil on the hemorheology index,carotid artery plaque and arteriosclerosis index(ASI) in elderly cerebral thrombus patients with vertebrobasilar insufficiency(VBI).Methods:A total of 80 elderly cerebral thrombus VBI patients were randomly divided into an observation group and a control group,with 40 cases in each group.The patients in the control group received conventional treatment and Danshen Chuanxiongqin injection,and those in the observation group were treated as the control group with addition of alprostadil for 14 days.The clinical effectiveness,hemodynamics and hemorheology index,ASI and latency changes of auditory brainstem response(ABR) were observed.Results:After 14 days of treatment,the total response rate in the observation group(92.50%) was significantly higher than that in the control group(70.00%)(P<0.05).The left vertebral artery Vm,the right vertebral artery Vm,and the basal artery Vm in the observation group were higher than those in the control group,and the plasma specific viscosity,erythrocyte aggregation index,TC,HDL-C,and ASI were significantly lower(P<0.05).The SF-36 quality of life scale(MCS,RE,RP,SF and GH) scores in the observation group were significantly higher than those in the control group,and the wave Ⅲ and Ⅴ incubation period as well as wave Ⅰ-Ⅲ,Ⅲ-Ⅴ andⅠ-Ⅴ interphases were shorter(P<0.05).Conclusion:Alprostadil could enhance the curative effect of early cerebral thrombosis VBI,improve the hemorheology index,stabilize carotid artery plaque and reduce ASI.
