摘  要:目的探讨富血小板纤维蛋白(PRF)联合引导骨再生技术(GBR)在牙周病患者颌骨缺损中精细位点重建的临床应用价值。方法选择颌骨骨量不足的牙周病患者126例,随机均分为对照组和观察组,分别给予GBR治疗和自体PRF联合GBR治疗,随访6个月。比较2组患者术后6 d修复体和牙龈情况及疼痛度,术后6个月骨缺损区骨密度及满意度。结果2组各失访2例患者,最终共纳入122例患者。2组患者术后6 d口腔黏膜瓣颜色异常率、肿胀率及疼痛度均无显著差异(P>0.05)。植入6个月后,观察组骨缺损区骨密度优的比例显著高于对照组(P<0.05),2组术后咀嚼功能以及美观度的满意度无显著差异(P>0.05)。观察组术后3、6个月修复体至牙槽嵴顶的距离显著短于对照组(P<0.05),修复体至牙槽嵴顶距离≤2 mm的比例显著高于对照组(P<0.05)。结论PRF联合GBR有利于促进牙周病颌骨缺损患者植骨治疗后骨组织修复及新生。AIM To explore the clinical value of platelet-rich fibrin(PRF)combined with guided bone regeneration(GBR)in the reconstruction of fine sites in patients with periodontal disease and jaw defects.METHODS A total of 126 periodontal disease patients with insufficient bone mass of jaw were selected and randomized divided into control group and observation group(n=63 in each group).The control group and observation group were treated with GBR and autologous PRF combined GBR respectively,followed up for 6 months.The condition and pain of the prosthesis and gums were observed at 6 days after operation.The bone mineral density and satisfaction of the bone defect area were observed at 6 months after operation.RESULTS Two patients were lost to follow-up in each group,and a total of 122 patients were included.There were no significant differences in oral mucosal color abnormality rate,swelling rate and pain between the two groups(P>0.05).After 6 months of implantation,the excellent ratio of bone density in the bone defect area of the observation group was significantly higher than that of the control group(P<0.05).There was no significant difference in postoperative chewing function and aesthetic satisfaction between the two groups(P>0.05).The distances from the prosthesis to the alveolar crest in the observation group were significantly shorter than that in the control group at 3 and 6 months after operation(P<0.05),and the ratio of the prosthesis to the alveolar crest distance≤2 mm was significantly higher than that of the control group(P<0.05).CONCLUSION PRF combined with GBR is beneficial to promote bone tissue repair and regeneration of bone graft reatment in jaw defects patients with periodontal disease and jaw defects treatment.
