摘  要:目的 探讨磁共振全身弥散加权成像(whole body- diffusion weighted imaging, WB-DWI)联合常规CT/MRI检查对肺癌病灶及其转移灶、淋巴结受累情况的诊断和鉴别诊断的增益价值。方法 回顾性分析15例经病理或随访证实的肺癌患者淋巴结或其他脏器及骨骼转移的WB-DWI扫描显像,并结合常规影像学检查(CT/MRI )资料。结果 15例肺癌患者转移性病灶均见明显弥散受限,其中6例(6/15,40%)出现淋巴结或其他脏器转移,CT发现4例;7例(7/15,47%)出现骨转移,CT/MRI发现5例;8例(8/15,53%)出现肺或其他脏器转移,CT/MRI均检出。结论 WB-DWI结合常规CT/MRI检查是诊断肺癌及其转移灶合理、有效、快捷的检查手段,具有较高的临床应用价值,WB-DWI对常规CT/MRI检查具有较高的增益价值,能早期发现转移性病变,在肺癌的全身转移的监测方面具有较大的优势。Objective To explore the gain values of combination of WB-DWI with regular CT/MR Scan in diagnosis and differential diagnosis of lung cancer primary and/or metastatic lesions. Methods We retrospectively analyzed the image data(combination of WB-DWI with Regular CT/MR Scan of lymphnodes, other organs and bone metastatic lesions in 15 lung cancer patients diagnosed by histopathology. Results Metastatic lesions in lymphnodes or other organs were found in 6/15(40%) lung cancer patients. They were diagnosed by CT Scan in 4/6 patients, metastatic lesions in bones in 7/15(47%) patients and by CT Scan/ MRI in 5/7 patients, and metastatic lesions in lung or other organs in 8/15(47%) patients. They were all diagnosed by CT Scan/MR. Conclusion It is reasonable,effective and fast method in diagnosis and stage of lung cancer by combination WB-DWI with regular CT/MR Scan, with high clinical application value. It is much better than by regular CT/MR Scan that can find metastatic lesions much earlier. It has much more advantages in monitoring metastatic lesions in patients with malignancy.
