摘  要:目的探讨比较2种不同拟合方法对健康志愿者肾脏IVIM参数稳定性和一致性的影响。方法对符合入组标准的20例健康志愿者采集双肾IVIM图像,利用后处理软件对图像运用2种不同拟合方法进行处理,分别获取IVIM参数值。由2名放射科医师分别独立评估并处理数据,其中1名医师间隔3月再次对数据进行处理。采用组内相关系数(ICC)和Bland-Altman图分析2名医师和1名医师间隔时间处理的数据一致性。利用变异系数(CV)评估2种拟合方法获得参数的稳定性,并采用配对t检验比较2种拟合方法获得的肾脏皮质、髓质各参数的差异。结果 2名医师和1名医师间隔时间运用2种拟合方法获得的IVIM参数,皮质的ICC值均高于髓质。2种拟合方法获得参数比较,分段拟合方式(IVIM-mono)获得各参数ICC值均高于全拟合方式(IVIM-bi)。另外,医师1间隔时间测得各参数的ICC值均高于2名医师测得参数的ICC值。Bland-Altman图显示2名医师和1名医师间隔时间2种拟合方法测得的参数纯水分扩散系数(D)值差值分布相对集中,皮质与髓质比较,以皮质更为集中。2种拟合方法获得参数比较,IVIM-mono获得皮质参数D值最为集中。IVIM各参数变异系数(CV)值比较,髓质CV值普遍较皮质高。IVIM参数D、灌注分数(Fp)值的CV值均低于伪扩散系数(Dp)值,D值的CV值最小。2种拟合方法获得参数比较,IVIM-mono获得的各参数CV值均低于IVIM-bi,以皮质DIVIM-mono最小(CV=3.97),髓质DpIVIM-bi最高(CV=68.48)。结论运用2种拟合方法对正常肾脏IVIM参数进行稳定性和一致性比较,IVIM-mono获得的参数稳定性和一致性普遍比IVIM-bi要好,而参数D的稳定性和一致性最好。Objective To investigate the effects of different fitting methods on stability and consistency of IVIM parameters in normal kidney.Methods IVIM images were collected from 20 healthy volunteers who met the enrollment criteria.Two different fitting methods were used to process the images by post processing software, and the IVIM parameters were obtained respectively. Data were independently assessed and processed by two radiologists,one of whom processed the data again at three months interval.Intra-class correlation coefficient (ICC) and Hand-Altman plot were used to analyze the data consistency.Coefficient of variation (CV) was used to evaluate the stability of the parameters obtained by the two fitting methods,and paired r-test was used to compare the differences of the parameters of renal cortex and medulla obtained by the two fitting methods.Results ICC values obtained by the two fitting methods of cortex were higher than those of medulla,and the parameters obtained by IVIM-mono fitting method were higher than those of IVIM-bi. In addition, the ICC values of each parameter measured by one radiologist were higher than those measured by the two radiologists.Bland-Altman plot showed that the distribution of difference in D values between the two fitting methods was relatively concentrated,and cortex was more concentrated than medulla.Upon comparison with the two fitting methods, the cortical parameter D value obtained by IVIM-mono was the most concentrated. Compared with the CV values of IVIM parameters, the CV values of renal medulla were generally higher than those of cortex. And for the specific parameters, CV values of Dp were higher than D and Fp,D was the smallest.The CV values of IVIM-mono were lower than those of IVIM-bi, with cortical Divim mono being the smallest (CV =3.97) and medullary DpiviMin being the highest (CV = 68.48).Conclusion In conclusion, thestability and the consistency of IVIM parameters obtained by IVIM-mono are generally better than those obtained by IVIM-bi,and the stability and the c
