摘　　要：目的了解宜昌市2005~2015年的肿瘤疾病负担情况及变化趋势,为针对性的开展肿瘤防控提供依据。方法利用中国分地市疾病负担研究数据,分析报告宜昌市肿瘤的死亡率,早死概率,过早死亡损失寿命年(YLL),伤残损失寿命年(YLD)和伤残调整寿命年(DALY)。结果2005~2015年宜昌市的肿瘤的标化死亡率和早死概率均呈逐年下降趋势。标化死亡率从1.76‰下降至1.54‰,过早死亡率从9.76%下降至7.79%。2015年标化DALY率排名前5位分别为肺癌、肝癌、胃癌、结直肠癌和子宫颈癌,分别为837.40/10万,697.48/10万,487.53/10万,258.38/10万,178.37/10万。在所有肿瘤中,子宫颈癌的疾病负担增长最快,标化YLL率从79.27/10万上升至171.81/10万,标化YLD率从3.19/10万上升至6.56/10万,标化DALY率从82.46/10万上升至178.37/10万。结论宜昌市肿瘤的标化死亡率和早死概率呈下降趋势,但肺癌、肝癌疾病负担严重,女性生殖系统的肿瘤变化较大,是干预和控制的重点所在。Objective To understand the disease burden caused by tumor in Yichang city during 2005-2015,so as to provide the evidence for the targeted prevention and control of tumor. Methods Using the data of disease burden study in different cities in China,the mortality of tumor,premature mortality,years of life lost(YLL),years lived with disability (YLD)and disability adjusted of life years(DALY)in Yichang city were analyzed and reported. Results From 2005to 2015,the standardized mortality and premature mortality of tumor in Yichang city showed a decreasing trend year by year.The standardized mortality decreased from 1.76‰to 1.54‰,and the premature mortality decreased from 9.76%to 7.79%.In 2015,the top 5cancers sorted by standardized DALY rate were lung cancer,liver cancer,stomach cancer,colorectal cancer and cervical cancer,the standardized DALY rate was 837.40/105,697.48/105,487.53/105,258.38/105 and 178.37/105 respectively.Among all tumors,the disease burden of cervical cancer grew the fastest.The standardized YLL rate increased from 79.27/105 to 171.81/105,the standardized YLD rate increased from 39.19/105 to 66.56/105,and the standardized DALY rate increased from 82.46/105 to 178.37/105. Conclusion The standardized mortality and premature mortality of tumors in Yichang city show a downward trend,but the disease burden of lung cancer and liver cancer is serious,female reproductive system tumors change greatly,which is the focus of prevention and control.
