摘 要:目的 探讨北京顺义某社区人群血清细胞间黏附分子-1(intercellular cell adhesion molecule-1,ICAM-1)及血管细胞黏附分子-1(vascular cell adhesion molecule,VCAM-1)与脑大、小血管病的相关性。方法 纳入2013年6月至2018年8月北京市顺义某地区5个村年龄≥35岁、独立生活、排除神经系统疾病病史的村民共1153人,采用Luminex液相芯片检测技术测定血清ICAM-1及VCAM-1水平,使用超声检查仪评估是否存在颈动脉斑块,采用磁共振成像仪评估颅内动脉粥样硬化性狭窄(ICAS)以及脑小血管病变指标腔隙、白质高信号体积(WMHV)及脑微出血(CMBs)情况。应用多重线性回归及Logistic回归分析探讨血清黏附分子与脑大、小血管病变的相关性。结果 1153名受试者平均年龄为(55.6±9.5)岁,男性占36.7%。在调整传统血管病危险因素等混杂因素后,ICAM-1、VCAM-1与颈动脉斑块及ICAS均不相关,但VCAM-1与WMHV(β= 0.190± 0.048, P <0.01)相关,VCAM-1、ICAM-1均与CMBs相关且ICAM-1对CMBs有弱保护作用。结论 细胞黏附分子与脑小血管病具有相关性,此结论有待后续大样本研究证实,以期为脑血管病发病机制的深入研究提供依据。Objective To investigate the associations between intercellular cell adhesion molecule-1(ICAM-1), vascular cell adhesion molecule-1(VCAM-1)and cerebrovascular diseases including cerebral large and small vessel diseases in a community based study in Beijing. Methods A total of 1153 participants aged 35 years or older,without neurologic diseases in Shunyi District,Beijing,were included,from June 2013 to August 2018.The levels of serum ICAM-1 and VCAM-1 were measured by Human Magnetic Luminex Assays.Carotid plaques were measured by ultrasonography.Intracranial atherosclerotic stenosis (ICAS)and small vessel disease markers including lacunes,white matter hyperintensity volume(WMHV)and cerebral microbleeds(CMB)were evaluated by MRI.Multivariate linear regression and Logistic regression analysis were used to investigate the correlation between the adhesion molecules and cerebrovascular diseases mentioned above. Results The average age of the participants was (55.6±9.5)years old,and males accounted for 36.7%. After adjusting for conventional vascular risk factors as the confounding factors,no correlation was found between the two adhesion molecules and large artery atherosclerosis.However,we found that VCAM-1 (β=0.190±0.048, P <0.01)was significantly correlated with WMHV,while ICAM-1 and VCAM-1 were associated with CMBs,and ICAM-1 had a weak protective effect on CMBs. Conclusions Cell adhesion molecules are associated with cerebral small vessel diseases.Further studies are still needed to confirm this conclusion,in order to provide scientific basis on the pathogenesis of cerebrovascular diseases.
