老年胃低分化腺癌患者外周血内皮素-1检测及临床意义探讨

摘　　要：目的探究外周血内皮素-1(ET-1)作为胃癌肿瘤标志物的临床价值。方法采用放射免疫法分别测定老年胃低分化腺癌患者(80例)、胃良性肿瘤患者(20例)术前与术后,及健康体检者(20例)血清中ET-1的表达水平,并探讨ET-1与胃低分化腺癌肿瘤浸润深度、淋巴结转移及临床分期的关系。结果(1)胃低分化腺癌组患者ET-1表达水平术前高于良性对照组及健康对照组,术后表达水平明显降低,但仍高于良性对照组(均P<0.01);(2)胃低分化腺癌组患者ET-1表达水平随着肿瘤浸润深度增加而升高,淋巴结转移阳性者的ET-1表达水平比阴性者高,临床分期Ⅲ期者的ET-1表达水平比Ⅰ~Ⅱ期者的高,差异均有统计学意义(均P<0.05)。结论外周血ET-1可作为老年胃低分化腺癌的患者肿瘤标志物,但是否可作为所有胃癌患者的肿瘤标志物尚需进一步深入研究。Objective To investigate serum levels of endothelin-1(ET-1)in patients with gastric cancer and its clinical significance.Methods Serum levels of ET-1 was detected by radioimmunoassay in 80 patients with poorly differentiated gastric adenocarcinoma,20 patients with benign gastric disease and 20 normal subjects.The changes of serum WT-1 levels before and after operation in gastric cancer patients,and the relationship of ET-1 levels with pathological stage,lymph node metastasis and clinical stage of gastric cancer were analyzed.Results The serum ET-1 levels in patients with poorly differentiated gastric adenocarcinoma were significantly higher than those in benign control group and healthy control group before operation(P<0.01).The ET-1 levels in gastric cancer patients were decreased significantly after operation(P<0.01),but still higher than those in benign control and normal groups(P<0.01).Serum ET-1 levels were positively correlated with the pathological stage,number of lymph node metastases and clinical stage(all P<0.05),as well as correlated with the clinical pathological stage(PTNM)after operation(P<0.05).Conclusion Serum ET-1 may be used as a tumor marker for diagnosis and evaluation of poorly differentiated gastric adenocarcinoma.
