甲状腺癌根治术和甲状旁腺自体移植术联合治疗甲状腺癌的应用研究

摘　　要：目的分析甲状腺癌根治术和甲状旁腺自体移植术联合治疗甲状腺癌的应用。方法方便选择2017年6月-2018年6月该院收治的160例甲状腺癌患者为研究对象,依照就诊顺序,将其随机平均分成对照组、观察组两组,每组80例。对照组接受甲状腺癌根治术,观察组接受甲状腺癌根治术+甲状旁腺自体移植术联合治疗,分析两组受试者手术后临床症状以及症状变化详情、PTH水平比较情况、永久性甲状旁腺功能减退发生率、短暂性甲状旁腺减退发生概率。结果观察组患者的感觉异常、腕-足痉挛、肢/指端麻木以及焦虑人数比例明显更低(χ^2=10.254、1.225、8.963、10.639,P<0.05),观察组PTH水平改善情况和对照组相比明显更好(P<0.05),观察组受试者永久性甲状旁腺功能减退发生率和短暂性甲状旁腺减退发生概率明显更低(χ^2=15.254、18.255,P<0.05)。结论对于甲状腺癌患者,积极实施甲状腺癌根治术联合甲状旁腺自体移植术进行治疗,安全性强,有效性高,该项手术能够全面防止患者出现永久性甲状旁腺功能减退。可促进患者疾病转归,因此值得进一步在临床中推广应用。Objective To analyze the application of thyroid cancer radical resection and parathyroid autograft in the treatment of thyroid cancer.Methods A total of 160 patients with thyroid cancer admitted to our hospital from June 2017 to June 2018 were conveniently selected.According to the order of treatment,they were randomly divided into two groups: control group and observation group,with 80 cases in each group.The control group underwent radical thyroidectomy,and the observation group received thyroid radical radical surgery + parathyroid autograft transplantation.The clinical symptoms and symptom changes,PTH level comparison,permanent parathyroid gland were analyzed.The incidence of hypofunction and the probability of transient hypoparathyroidism was collected.Results The proportion of sensory abnormalities,wrist-ankle,limb/finger numbness,and anxiety was significantly lower in the observation group compared with the control group(χ^2=10.254,1.225,8.963,10.639,P<0.05).The improvement of PTH level in the observation group was significantly better,.Compared with the control group(P<0.05),the incidence of permanent hypoparathyroidism and the incidence of transient hypoparathyroidism were significantly lower in the observation group(χ^2=15.254,18.255,P<0.05).Conclusion For patients with thyroid cancer,active thyroidectomy combined with parathyroid autograft is safe and effective,and the operation can prevent patients from appearing permanently hypoparathyroidism.It can promote the patient's disease outcome,so it is worth further application in the clinic.
