摘 要:目的通过牙周袋内局部用药,比较天然草本类姜黄素与合成类氯己定治疗慢性牙周炎的疗效。方法采用PubMed、Embase、Cochrane Library等3个电子数据库,对相关文献进行系统检索。仅纳入对比姜黄素与氯己定在菌斑指数(PI)、牙龈指数(GI)、探诊深度(PD)和临床附着水平(CAL)等牙周治疗评价指数方面的随机对照试验(RCT)。RCT参与者限定为慢性牙周炎患者。用药方式为牙周袋内局部应用缓释凝胶或缓释片。纳入研究包括短期应用(<4周)和长期应用(≥4周)。评价结局为局部或全口应用、短期或长期应用姜黄素或氯己定在减少PI、GI、PD和CAL上的区别。结果检索到36篇文献,21篇符合纳入标准,最后有6篇纳入meta分析。meta分析结果表明:随访时间小于1个月时姜黄素减少CAL的疗效明显优于氯己定,但减少PI、GI、PD的疗效与氯己定比较,差异均无统计学意义(P>0.05);随访时间大于或等于1个月时姜黄素减少PI、GI、PD的疗效与氯己定相似,长期疗效比较,差异均无统计学意义(P>0.05)。结论姜黄素可作为氯己定的替代药物,用于消除牙龈和牙周组织炎症,但尚需进行更多高质量研究进一步探索2种药物在治疗牙周病方面的疗效。Objective To compare the efficacy of natural herbal curcumin with synthetic chlorhexidine in the treatment of chronic periodontitis.Methods Three electronic databases,such as PubMed,Embase,and Cochrane Library,were used to systematically search related documents.Only randomized controlled trials(RCT) comparing the evaluation index of periodontal treatment with curcumin and chlorhexidine in plaque index(PI),gingival index(GI),depth of probing(PD),and clinical attachment level(CAL) were included.RCT participants were defined as patients with chronic periodontitis.The method of application was a topical application of a sustained release gel or a sustained release tablet in a periodontal pocket.Inclusion studies included short-term use(<4 weeks) and long-term use(≥4 weeks).Evaluating the outcomes was the difference in decreasing PI,GI,PD and CAL between the topical or full-mouth applications and between short-term or long-term use of curcumin or chlorhexidine.Results Thirty-six articles were retrieved,21 articles met the inclusion standard,and six articles were included in the meta-analysis.The meta analysis results showed that:when the follow-up time was less than 1 month,curcumin was significantly better than chlorhexidine in the effect for reducing CAL.However,there was no statistically significant difference in the efficacy for reducing PI,GI,and PD compared with chlorhexidine( P >0.05).When the follow-up time was greater than or equal to 1 month,the curative effect of curcumin on reducing PI,GI,and PD was similar to that of chlorhexidine.There was no statistically significant difference in long-term efficacy( P > 0.05).Conclusion Curcumin can be used as an alternative to chlorhexidine to eliminate inflammation of the gums and periodontal tissues,but more high-quality studies are needed to further explore the efficacy of the two drugs in the treatment of periodontal disease.
