摘　　要：目的探讨乳管镜在乳管内肿瘤切除术中的应用价值。方法选择2017年5月到2019年4月在甘肃省妇幼保健院乳腺科诊治的乳管内肿瘤患者78例,根据随机数字表法分为观察组与对照组,各39例。对照组给予传统腺体切除术,观察组给予乳管镜下腺体切除术,记录与调查两组预后。结果两组的手术时间对比差异无统计学意义(P>0.05),观察组的切口长度、切除标本重量都显著少于对照组(P<0.05)。术后1个月观察组的总有效率为100.0%,显著高于对照组的87.2%(P<0.05)。观察组术后1个月的皮瓣积液、皮瓣坏死、切口感染、上肢淋巴水肿等并发症发生率为5.1%,显著低于对照组的30.8%(P<0.05)。术后随访3个月,观察组的躯体、心理、社会与精神领域等生活质量评分都显著高于对照组(P<0.05)。结论乳管镜在乳管内肿瘤切除术中的应用能减少对患者的创伤,降低术后并发症的发生,提高治疗效果,改善患者的生活质量。Objective To investigate the value of breast ductoscopy in the treatment of intraductal tumor resection.Methods From May 2017 to April 2019,78 patients with intraductal tumors were selected and treated in our hospital and were equally divided into the observation group and the control group accorded to the random number table method.The control group were given conventional glandectomy,and the observation group were given cystoscopic glandectomy.The prognosis of the two groups were recorded and investigated.Results There were no significant difference in the operation time between the two groups(P>0.05).The length of the incision and the weight of the resected specimens in the observation group were significantly lower than those in the control group(P<0.05).The total effective rates of the observation group at 1 month after surgery were 100.0%,which were significantly higher than that of the control group(87.2%)(P<0.05).The incidence of complications such as flap effusion,skin flap necrosis,wound infection,and upper limb lymphedema in the observation group were 5.1%,which were significantly lower than that of the control group(30.8%)(P<0.05).After 3 months of followed-up,the quality of life scores in the physical,psychological,social and spiritual fields of the observation group were significantly higher than those in the control group(P<0.05).Conclusion The application of breast ductoscopy in intraductal tumor resection can reduce the trauma of patients,reduce the incidence of postoperative complications,improve the therapeutic effect and improve the quality of life of patients.
