基于五运六气理论探析吴鞠通辨治温毒临证经验

摘　　要：目的:从五运六气理论探讨吴鞠通辨治温毒临证经验,为临床诊治温毒及类似病证提供参考。方法:将《吴鞠通医案》中符合纳入标准的温毒医案录入中医传承辅助平台数据库V2.5系统,并进行药物频次、症状频次及用药模式的数据挖掘。结果:符合条件的温毒病例共21例,所载温毒病例多在1833年、1804年和1828年,其运气形势与“火、风、燥”相关。共出现症状54个,包括谵语、口渴、思凉饮、面赤、不大便、肢厥、身痛、颊肿、喉肿痛、瘰疬、脉伏、潮热等。共出现中药64味,主要有玄参、生石膏、麦冬、生地黄、桔梗、金银花、连翘、芦根、人中黄、炙甘草、牛蒡子、知母、马勃、牡丹皮等。结论:吴鞠通辨证治疗温毒遵循运气形势,其临证经验值得在临床类似病证中应用。Objective:To discuss WU Jutong’s clinical experience in syndrome differentiation and treatment of warm toxin from the theory of five circuits and six qi,in order to provide reference for clinical diagnosis and treatment of warm toxin and similar diseases and syndromes. Methods:The medical cases of warm toxin in WU Jutong Yi’an,and in accordance with the inclusion criteria,were input into the Traditional Chinese Medicine Inheritance Support System V2.5;the data mining of frequency of medicine as well as symptoms and administration patterns was conducted. Results:There were 21 cases of warm toxin in accordance with the conditions.Most of recorded cases were in 1833, 1804 and 1828. Their circuits-qi were related to " fire, wind and dryness". There were 54 symptoms such as soliloquy,thirst,desire for cool beverages,reddish complexion,no stool,reversal cold of the extremities,pain in the body, buccal swelling, swelling and pain of the throat, scrofula, hidden pulse, tidal fever and so on. There were 64 Chinese herbs, such as Radix Scrophulariae, Gypsum Fibrosum, Radix Ophiopogonis, Radix Rehmanniae, Radix Platycodonis, Flos Lonicerae Japonicae, Fructus Forsythiae, Rhizoma Phragmitis, pulvis glycyrrhizae praeparatus, Radix et Rhizoma Glycyrrhizae Praeparata cum Melle,Fructus Arctii,Rhizoma Anemarrhenae, Lasiosphaera seu Calvatia, Cortex Moutan and so on. Conclusion:WU Jutong followed the circuits-qi in treating warm toxin based on syndrome differentiation and treatment,whose clinical experience is worthy of clinical promotion and application in similar diseases and syndromes.
