摘 要:目的:探讨高尿酸血症(HUA)对非酒精性脂肪肝(NAFLD)患者血管内皮功能及颈动脉粥样硬化的影响。方法:将临床确诊的120例NAFLD患者据血尿酸水平分为高尿酸组(HUA组)63例、正常尿酸组(NUA组)57例,分别检测血清一氧化氮(NO)和内皮素-1(ET-1)的水平、肱动脉内皮功能及颈动脉内-中膜厚度(IMT),对各组数据进行对比分析。结果:HUA组血清NO水平、肱动脉内皮依赖性舒张功能(FMD)、非内皮依赖性舒张功能(NMD)均较NUA组显著降低(均P<0.05),血清ET-1水平、斑块检出率显著升高,IMT显著增厚(均P<0.05)。结论:HUA可加重NAFLD患者内皮功能损伤和动脉粥样硬化程度,增加发生心脑血管事件的风险。Objective:To investigate the effects of hyperuricemia (HUA) on vascular endothelial function and carotid atherosc- lerosis in patients with nonalcoholic fatty liver disease (NAFLD). Methods:A total of 120 patients with NAFLD were divided into two groups,63 cases with high uric acid of HUA group and 57 cases with normal uric acid of NUA group,to detect serum nitric oxide (NO) and endothelin-1 (ET-1) levels,brachial artery endothelial function and carotid artery intima-media thickness (IMT) respectively. Groups of data were measured and compared. Results:Serum NO level,brachial artery flow-mediated dilation (FMD) and nitroglycerin-mediated dilation (NMD) in HUA group were significantly lower than those in NUA group (P<0.05),the differences were statistically significant. Compared with NUA group,serum ET-1 level,plaque detection rate and IMT in HUA group increased significantly (P<0.05),the differences were statistically significant. Conclusion:HUA aggravates the degree of endothelial dysfunction and atherosclerosis in NAFLD patients and increases the risk of having cardiovascular and cerebrovascular events.
