摘 要:目的 观察不同营养治疗对同步放化疗中晚期食管癌患者营养状况、短期疗效与毒副作用的影响。方法 选取食管鳞癌营养风险患者60例,随机分为治疗组1和治疗组2,每组各30例,常规给予食管癌同步放化疗(三维适形调强放疗联合TP方案每周化疗),治疗组1给予一般营养治疗(营养宣教、日常饮食与口服营养液),治疗组2严格按照125.57kJ/(kg·d)的标准进行营养治疗;同时筛选历史入院同条件但未进行营养治疗的30例患者为对照组。治疗结束后对患者营养状况、短期疗效及毒副作用进行统计分析。结果 治疗组1与治疗组2体质量丢失低于对照组(P<0.05),治疗组1、治疗组2血红蛋白浓度、血清白蛋白水平均高于对照组(P<0.05);三组患者近期疗效比较,差异无统计学意义(P>0.05);治疗组1与治疗组2急性食管炎发生率较对照组低(P<0.05)。结论 营养治疗可改善中晚期食管癌患者同步放化疗的营养状况,并降低食管炎的程度。Objective To observe the effects of different nutritional treatments on nutritional status,short-term efficacy and side effects of patients with advanced esophageal cancer during concurrent chemoradiotherapy. Methods 60 patients with nutritional risk of esophageal squamous cell carcinoma were randomly divided into treatment group1 and treatment group2,30 cases in each group.The concurrent chemoradiotherapy for esophageal cancer (three-dimensional conformal intensity-modulated radiotherapy combined with TP regimen weekly chemotherapy) was given routinely.Treatment group1 was given general nutrition therapy (nutrition education,daily diet and oral nutrient solution),and treatment group2 was strictly treated according to the standard of125.57 kJ/(kg·d).At the same time,30 patients with the same conditions but no nutritional treatments were selected as the control group.After the treatment,the patient′s nutritional status,short-term efficacy and side effects were statistically analyzed. Results The body mass loss of treatment group1 and treatment group2 was lower than that of the control group ( P < 0.05 ).The hemoglobin concentration and serum albumin level in the two group were higher than those in the control group ( P < 0.05).There was no significant difference in the short- term efficacy between the three groups ( P > 0.05 ). The incidence of acute esophagitis in the treatment group1 and treatment group2 was lower than that in the control group ( P <0.05). Conclusion Nutritional therapy can improve the nutritional status of concurrent chemoradiotherapy in patients with advanced esophageal cancer and reduce the degree of esophagitis.
