低分割放疗联合多西他赛治疗70岁以上老年晚期非小细胞肺癌临床观察

摘　　要：目的探讨低分割放疗联合多西他赛治疗老年晚期非小细胞肺癌的临床疗效。方法收集接受胸部低分割放疗晚期非小细胞肺癌共53例,包括联合多西他赛化疗28例,单纯放疗25例。放疗方案:单次剂量2.5Gy,5次/周,总剂量30~50Gy。多西他赛,25mg/m^2,放疗前一天开始,1次/周,共2~4次。结果随访3~28个月,中位随访时间11个月。低分割组和同步放化组肺部病灶近期疗效(CR+PR)分别为64%和67.9%;1年生存率分别为44%和50.6%,两年生存率分别为8%和10.7%,中位生存时间分别为10和11个月;放射性食管炎发生率52%(13/25)和53.6%(15/28),一、二级放射性肺炎发生率32%(8/25)和35.7%(10/28)。结论三维适形低分割放疗联合多西他赛治疗高龄老年晚期非小细胞肺癌疗效确切,副作用尚可耐受。Objective To investigate the clinical efficacy of weekly dose of docetaxel With concurrent hypofractionated high-dose radiotherapy in treatment of elderly patients with advanced non-small cell lung cancer.Methods A total of 53 patients with advanced non-small cell lung cancer who underwent low-segment radiotherapy were enrolled,including 28 patients receiving combined docetaxel chemotherapy and 25 patients receiving radiotherapy alone.Radiotherapy regimen: single dose 2.5Gy,5 times/week,total dose 30~50 Gy.Docetaxel,25 mg/m^2,starts one day before radiotherapy,once a week,a total of 2 to 4 times.Results Followed up for 3~28 months,the median follow-up was 11 months;Hypofractionated high-dose radiotherapy and chemo-radiotherapy in short term efficacy(CR+PR):64% and 67.9%;1-year survival rate of 44 % and 50.6%,2-year survival rate of 8% and 10.7%,median survival time 10 and 11 months.Radiation esophagitis incidence of 52%(13/25)and 53.6%(15/28).The second class above radiation pneumonia rate was 32%(8/25)and 35.7%(10/28).Conclusion Curative effect of weekly dose of docetaxel with concurrent hypofractionated high-dose radiotherapy in treatment of elderly patients with advanced non-small cell lung cancer is definitely,radiation side effects can be tolerated.
