摘  要:目的探讨程序性死亡分子1(PD-1)、程序性死亡配体1(PD-L1)和T淋巴细胞亚群在乳腺癌中的表达及价值。方法选取2017年6月至2018年6月该院收治的乳腺癌患者92例,检测肿瘤组织中PD-1、PD-L1、CD4^+T淋巴细胞、CD8^+T淋巴细胞和调节性T淋巴细胞(Treg),并分析其与乳腺癌患者临床特征的相关性。结果与癌旁组织比较,乳腺癌组织PD-1、PD-L1、Treg、CD4^+T淋巴细胞表达增高,而CD8^+T淋巴细胞表达降低(P<0.01)。与TNM分期为Ⅰ或Ⅱ期的患者比较,Ⅲ期的患者乳腺癌组织PD-1、PD-L1、Treg、CD4^+T淋巴细胞表达增高,而CD8^+T淋巴细胞表达降低(P<0.01)。与肿瘤直径小于5cm的患者相比,大于或等于5cm的患者乳腺癌组织PD-1、PD-L1、Treg、CD4^+T淋巴细胞表达增高,而CD8+T淋巴细胞表达降低(P<0.05)。结论PD-1和PD-L1在乳腺癌组织中升高,抑制T淋巴细胞对肿瘤细胞的杀伤,与TNM分期和肿瘤直径有关。Objective To explore the expressions and values of PD-1,PDL-1 and T lymphocyte subsets in breast cancer. Methods From June 2017 to June 2018,92 patients with breast cancer were collected in our hospital.The levels of PD-1,PD-L1,CD4^+T cells,CD8^+T cells and Tregs in tumor tissues were detected,and their correlation with clinical characteristics of breast cancer patients were analyzed. Results When compared with the adjacent tissues,the expression of PD-1,PD-L1,Tregs and CD4^+T cells increased in breast cancer tissues,while the expression of CD8^+T cells decreased (P<0.05).When compared with the patients with TNM stage Ⅰ or Ⅱ,the expression of PD-1,PD-L1,Tregs and CD4^+T cells in stage Ⅲ breast cancer tissues increased,while the expression of CD8^+T cells decreased (P<0.05).The expression of PD-1,PD-L1,Tregs and CD4^+T cells in breast cancer tissues (≥ 5 cm) of patients were higher than those of patients with tumor diameter < 5 cm,while the expression of CD8^+T cells was lower (P<0.05). Conclusion PD-1 and PD-L1 are elevated in breast cancer tissues and inhibit T cell killing,which is related to TNM stage and tumor diameter.
